On 5/15/2020 at 10:00 AM, Licensing Program Analyst (LPA) Helen Estrella arrived at 3645 Torrance Blvd, Torrance CA 90503 and met with Administrator Kryollos Wassily and Priest Angelos Guirges for an announced visit to conduct a pre-licensing inspection. The facility is requesting a capacity of 12 infant, ages 0 to 2 years old. The program will operate from Monday to Friday from 7:00 a.m. to ^:00 p.m. The applicant has a fire clearance from Inspector Sanders of Torrance Fire Department as of 05/12/20.

The facility consists of seven (7) classrooms, kitchen, one (1) staff bathroom, and two (2) children's restrooms. The applicant is requesting to use three (3) of the seven (7) classrooms, staff bathroom, and kitchen for the purpose of the infant license. (The facility is pending licensure for an increase in capacity for St. Abraam Preschool # 197415439 and will be requesting to use the additional rooms for the preschool license).The outdoor activity space(s) is located in the rear of the facility, across the walking path of the parking lot. There are two enclosed play yards. One play yard is licensed for St. Abraam Preschool. The play yard that is requested for the infant center is located across the walking path of the church and consists of a playground area with a play structure that is suitable for preschool children. The licensee making the play structure off limits for infants during outdoor play time. The Director shall ensure that gates are secure so that the walking path leading to the parking lot is inaccessible to children in care.

Access to the Facility
Parents will park in the parking space on the side of the facility. Parents will enter through the front of the property to the entrance of the building via staff access.

Sign in and sign out
Sign in/out will be conducted in each classroom and the applicant is informed to ensure that the person who signs the child in/out use his/her legal signature and shall record the time of day.

Meal/Snacks/Medication
The facility will provide AM/PM snacks, Lunch meals will be provided by parents. In the event a child does not have a lunch and the facility will have food on premises to meet the child's needs. Allergy list are shall be posted in each classroom and the kitchen. Water pitchers shall be made available y available for outdoor play.

LPA provided consultation with the licensee regarding the facility observation and evaluation. LPA measured the inside and outside of the facility. LPA informed the licensee that a subsequent pre-licensing inspection was required and for the facility to ensure that the facility including classrooms and play area were set up for licensure in order to accurately determine corrections and measurements. Licensee was informed that the following measurements taken today were based on the current observations of the facility. Upon determination of the subsequent visit, re-measurements may be required. The facility is requesting a waiver for the shared outdoor space usage.

The following corrections are needed:
  • Furniture & age appropriate equipment & in good repair
  • Toys to be safe, with no sharp edges, splinters or points, nor made of small pasts that can be pulled off and swallowed.
  • drinking water readily available
  • Separate area for isolation & care of ill children that has a mat, cot or crib for each ill child
  • outdoor play equipment & material to be developmentally appropriate for age group and not made of or contain toxic materials or substances.
  • Playground to be free of miscellaneous debris or hazards such as tree branches, cans, glass or bird droppings
  • High Chairs or feeding tables and ensure they have broad base legs. Ensure seats are in good repair and trays lock onto chairs
  • Changing tables have at least 1 inch thick padding covered with washable vinyl or plastic. Ensure that sides raised minimum 3 inches.

  • Changing table shall be within arm's length of a hand-washing station
  • Minimum of one (1) potty chair or toilet for every five (5) infants in toilet training
  • Cribs or other appropriate napping equipment available for each crib age infant. (Please refer to Crib Safety Act Implementation Instructions in 98APX-03 Errata, 1997 Chaptered Legislation & Implementation Summary)
  • Bedding & other infant napping shall be separately identified and stored for each infant
  • Area for each of child's belongings
  • Board with required postings
  • Incidental Medical Services (IMS) storage and re-evaluation
  • Outlet covers throughout the facility
  • Carbon Monoxide detector in each classroom and kitchen
  • Smoke Alarm in facility
  • 2A10BC Fire Extinguisher
  • First Aid Kit that includes Tweezers, Thermometer and Scissors

Based on this inspection the facility shall be recommended for licensure pending a subsequent inspection, corrections, management review of outdoor waiver and approval.

An exit interview was conducted and a copy of this report was provided to the Administrator.
