1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/06/2022 at 1:05pm Licensing Program Analyst (LPA), Denise Do Amaral Miranda conducted an unannounced case management inspection to the facility to ensure the health and safety standards as required governing California family child care homes. LPA met Houba Assaad, Licensee and discussed the reason of the visit. Licensee guided LPA on a tour (inside and outside) of the home. LPA confirmed with Licensee that the all adults residing/working in the home have criminal record/TB. There were 07 children in care (4 whom are infants) at the time of inspection with the licensee, and two Licensee’s Assistant. On 4/01/2022 Licensee submit to the El Segundo Regional Office a new sketch and a letter saying that bedroom#2 (sick area), bedroom#3 (daycare room#1), bedroom#4 (daycare room#2) and bathroom # 3 and #4, converted office room in to kitchen#2 located at the downstairs (bottom floor) of the house will be on limit. Areas identified on the facility sketch were inspected. The facility is a two-story home that includes: -1 master bedroom, 3 bedrooms, 5 bathrooms, 1 living room, 1 dining areas, and kitchen, front yard, and back yard with attached garage converted in a recreation family room and (dining room, living room#1) previously play area room locate by the main entrance door. The new primary care area is located on the bottom floor of the house. Per licensee and sketch revised, the licensee, will be provide care and supervision at the bedroom#3 and #4 (daycare room#1 &#2), bedroom#2 (sick area), children will use the bathroom#3 and #4, also, children will have access to the family room and backyard outdoor play area.

LPA observed that the office that was located at the bottom floor, by the family room (on limit) was removed and a kitchen #2 was installed with stove, refrigerate, table, sink and cabinets.
Per Licensee, the parents and children will use the gate located at the right-side of the house to access the facility.
Per Licensee and sketch provided on 4/1/2022, the top floor of the house will be off-limit to the children in care that is included the balcony area, kitchen#1, dinette area, dining room, living room#1&2, master bedroom, bathroom #1 &2, pantry.

Off-limit: Bottom floor (downstairs): Recreation Family Room, Laundry room, Hallway to access the recreation family room, storage, kitchen#2, bathroom 5.

LPA observed a swimming pool and jacuzzi located at the backyard of the facility. LPA observed a fence is in good repair and completely surrounding the pool. Fence is designated and installed so children could not remove any portion it.
The fence is at least five feet high. The fence is constructed so that the fence does not obscure the pool from view. No window or door provides direct access to the pool area. The gate is swings away from the pool, latching device located no more than four inches from the top of the gate. Bottom of fence is no more than 4 inches from the ground on hard surface. A jacuzzi is located beside of the swimming pool area.
LPA observed water inside of the swimming pool and jacuzzi, both are off-limit area.

The home has age appropriate toys. Electrical outlets have child protective covers in place making them inaccessible to children. Poison or dangerous items are stored/locked in cabinets and made inaccessible. Licensee states there are no firearms in the home and LPA observed no weapons at the time of this inspection. LPA observed two cats at the living room (off-limit area). During this inspection, no children were observed using the downstairs area.

LPA Miranda recommended upon completion of the following corrections:

DOWNSTAIR/Bottom floor:

Place latches at the following doors:
Closet doors located at the Sick room, day care room#1 and #2, Hallway door (giving access to the recreation family room (off-limit), storage room located by the family room.
Place latch at the kitchen#2 and bathroom’s#3& 4 cabinets and drawer
Licensee will remove the shampoos, lotions, from the bathroom#3 &#4 making inaccessible to the children in care.
Make provision for securing rolling portable items: Television and refrigerator located at the family room and a piano located at the child Room#3.

Per Licensee, she agreed to complete the recommendation no later than 04/11/2022 and submit photos of the recommendation via email to LPA Miranda.

Exit interview was conducted with Houba Assaad, Licensee, A copy of this report along with notice of visit was provide to licensee.
