On 04/26/2024 Licensing Program Analyst (LPA) Lisa Clayton arrived at the home to conduct an Unannounced Required 3-year Inspection. LPA Clayton was greeted by Licensee Houda Assaad. LPA Clayton observed 7 children being supervised and cared for appropriately by licensee and fingerprint cleared assistant. Hours of operation are 9am – 5pm. Licensee provides lunch, snacks, dinner, water and juice.

The home was toured with the Licensee for a health and safety inspection. The home is neat and clean with heating and ventilation for safety and comfort.

This is a two-story home that includes: 4 bedrooms, 4 ½ bathrooms, living room, formal dining room, kitchen/family room converted attached garage, bonus room, fenced front and back yard.



The ON LIMIT areas are as follows: Downstairs - bonus room (main daycare room), bedroom #2 (day care nap room) bedroom #3 (day care nap room) bathroom #3, and fenced backyard. The ISOLATION AREA will be in the daycare nap room.

The OFF-LIMIT AREAS are as follows: Downstairs – kitchenette, bedroom #4, the converted garage (licensees son’s living quarters) and the entire first floor (bedroom #1, bathroom #1, ½ bath, formal dining room, kitchen/family room, and patio), all of which will be inaccessible by child safety gates, closed and/or locked doors and visual supervision.

Per Licensee, there are no firearms in the home. Any poisons, detergents/cleaning compounds, medication and hazardous items that can pose a danger to children are inaccessible.

The home has a fully charged 2A:10: BC Fire Extinguishers, and a working smoke detector/carbon monoxide detector downstair in the day care room. LPA confirmed the phone number 310-559-0697.

There is an in-ground swimming pool that is fenced and in compliance with the following Title 22 Regulations: (e) All licensees shall ensure the inaccessibility of pools, including swimming pools (in-ground and above-ground), fixed-in-place wading pools, hot tubs, spas, fishponds or similar bodies of water, through a pool cover or by surrounding the pool with a fence. (1)Fences shall be at least five feet high and shall be constructed so that the fence does not obscure the pool from view. The bottom and sides of the fence shall comply with Division 1, Appendix Chapter 4 of the 1994 Uniform Building Code. In addition to meeting all of the aforementioned requirements for fences, gates shall swing away from the pool, self-close and have a self-latching device located no more than six inches from the top of the gate.

LPA reviewed 5 children’s files and observed them to be in compliance as they contained current contact information for authorized representatives and/or relatives who can assume responsibility for the child, and signed Parent’s Rights. LPA Clayton observed Individual Sleep Plans forms and Sleep logs for infants in care.



Licensee and staff have current CPR/First Aid certificates. LPA Clayton reminded licensee that Mandated Reporter training certificates are to be renewed every 2 years. LPA Clayton advised Licensee to have copies of immunizations required for childcare providers available for review by the Licensing Department.

LPA Clayton reminded licensee to access the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and­resources/safesleep as an additional resource. LPA Clayton informed applicant of the importance of checking for recalled infant devices on the United States Consumer Products Safety Commission (CPSC) website at https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on the purchased equipment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of 100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&R) throughout California.


During the exit interview, Licensee Houda confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO profile in FAS.

Per Title 22 Regulations and Health and Safety Codes, no Deficiencies were cited today.

An exit interview was conducted, a copy of this report was read and provided to licensee Houda and will be made available to the public upon request.

LIC 9213 Notice of Site Visit was provided and required to be posted for 30 days.
