25 | On 07/14/2021 12:35pm Licensing Program Analyst (LPA), conducted an unannounced Annual Required Inspection and was met by Licensee, Eliana Gallegos. Days and hours of operation are from Monday through Friday 7:00am to 6:00pm.
LPA toured the home inside and outside and a census was taken. Current facility sketch reviewed, and an updated sketch floor plan and yard will need, licensee will provide no later than 07/16/2021. Licensee confirmed that the front house is a is a single story, 3 bedrooms, 1 bathroom, living room, kitchen and attached garage. The front yard is not fenced and will be off limits to the children. The children will play in the back yard where it is fully fenced.
The off limit areas of the home include area are the bedroom#1,#2 and #3 (The third bedroom is located inside of the bedroom #2) and the attached garage. Per licensee the back house is off limit and the adults that are leaving on the back house has been associated to the facility.
The on-limit area are the living room, kitchen, bathroom and backyard.
The premises has the second house that located on the back of the house. The back house has one bedroom, bathroom and kitchen. |
