On 03/28/2022 at 7:30 am, Licensing Program Analyst (LPAs) Denise Miranda and Suzette Ornelas arrived at 716 Westmount Dr, West Hollywood, CA 90069. Applicant submitted an Licensee for an increase in capacity. On 03/03/2022, Licensee was granted a fire clearance by the County of Los Angeles Fire Department.
Present at the time of the inspection were Shauna Ricks-Scalmado, Licensee, Licensee’s assistant and Licensee’s husband Nicolas Scamaldo. LPA verified that all adults present in the home have obtained criminal record clearances and are associated to the facility. LPA reviewed the facility sketch (indoor and outdoor) and it was observed match the physical plant of the home.
LPAs Miranda and Ornelas met with Licensee Shauna Ricks-Scalmado and was guided on a tour of the indoor and outdoor of the home. LPA's observed 3 infants and 3 preschool children with licensee and licensee’s assistant.
The home is a one story a duplex detached house that includes:
1 bedroom, 2 bathrooms, living room, daycare room (dining room), kitchen, den and attached garage.
The following areas are designated as follows:
** day care room and living room– This is where main care will be conducted. LPA observed the day care room to have age appropriate furniture, games, learning materials and activities for children in care. Furniture was observed with no loose or sharps parts, clean and in good repair.
The main day care area shall be conducted in the living room and daycre room (dining room) for daytime activities. Children also have access to bathroom #1. LPAs also observed between kitchen and living room safety gates installed making the kitchen area inaccessible to children in care.
Chemicals, cleaning supplies, and poisons are kept in the laundry room are inaccessible

to children. The back yard shall be made accessible to day care children for outdoor activities. All areas of the home were inspected according to the LIC 999A- facility sketch.
**Bathroom#1 – located next to the day master bedroom.

** Backyard/play area located on the back of the house, between the entrance door of the day care room and attached garage. The outdoor area/back yard is fenced in. Licensee is stating that when children are having outside time she will ensure 100% supervision and never leave children unattended.

Areas that will be used by children were inspected for safety, comfort, cleanliness, telephone service, ventilation and central heater. Applicant completed health preventive and safety training and has current pediatric CPR, first aid certificate.


Off limits to day care children:
***Master bedroom and master bathroom, kitchen, front yard, den and attached garage. Per licensee children will not have access to this area.
Per Licensee, the garage will be close during day care hours and children will not have access to this area. Licensee was advised and licensee agreed that no child shall take naps or be left in a car seat at any time.
Per Licensee, no bodies of waters on the premises. There are no firearms or ammunition on the premises.
All poisons are kept in a locked storage area. No poisons were observed during the inspection. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible.

LPAs Miranda and Ornelas observed the following:
*Fire safety requirements: Fully charged fire extinguisher (Classification: 3A:40B:C), smoke and carbon monoxide detectors present and operable.
*Toys and equipment are observed and in good repair.
* The home is equipped with a first-aid kit: Cleansing pads, band aid, bandages, gauze, and a digital thermometer.

The following items were thoroughly discussed with the applicant:

*The necessity of earthquake safety and drills twice a year
*The role and responsibilities of being a mandated reporter
*Mandatory Forms for the children’s files and provider’s files
* LIC 9149 Landlord Consent Form, if you plan to care for more than 6 for Small or 12 for Large. Licensee needs Landlord consent to operate with capacity of 14 children.

*Licensee was referred to LIC 311D: Records to be Maintained at The Facility - Family Child Care Home.
*Licensee was made aware that it is their responsibility, as well as anyone who assists in providing care, to know Title 22 Regulations.
*Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.
Licensee must have the facility’s phone number at all times. If the phone number has changed, licensing must be notified.
*Regulation prohibits the smoking of tobacco on the premises.
*State law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category.
* All infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome) .

*Complaints shall be reported by applicant/licensee and/or parents to the complaint hotline at (844) 538-8766 and for general information and incident reporting; contact their local childcare office.

Licensee was also informed that the provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome.

*Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

LPA discussed the influenza waiver during the inspection. *Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Website: www.mandatedreporterca.com Applicants were reminded of their responsibility to report suspected child abuse. Reporting requirements: Applicant must report any unusual incident or injuries to the Child Care Regional office by telephone within 24 hours and in writing within 7 days. Applicant was provided with LIC 624 as a reference.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all

Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Assembly Bill 633: Upon receipt by the licensee, licensees are to provide to parents/guardians the following: Copies of any licensing reports that document a Type A citation- this includes facility visits and substantiated complaint investigations; copy of licensing documents pertaining to a conference conducted by a local licensing agency management representative and the licensee of this family child care home in which issues of noncompliance are discussed or copies of a summary of an accusation indicating the Department's intent to revoke the facility's license. Copies of any of the above licensing documents the licensee has received in the prior 12 months shall be provided to parents/guardians of newly enrolled child at the facility.

Senate Bill 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to: Department of Social Services | El Segundo Child Care Office | 300 N. Continental Blvd., Suite 290-A |El Segundo, CA 90245.

New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA reviewed a sample of children’s files and observed that all children’s file need the blue card CDPH 286, form files were complete with emergency information as required. Licensee’s pediatric CPR/First Aid expires on 04/2022.
A review of records indicates that all employees and/or volunteers have immunization records on file for influenza, pertussis and measles.

LPA obtained an updated copy of LIC9040 Children's roster, monthly schedule of children's attendance and a declaration regarding high chairs use.

The following is needed priory to licensure:

Licensee will stabilize the safety gate place to make inaccessible the cables/outlets located at the living room.
Provide a copy of the CDPH 286 – for all enrolled children in care.
Licensee will conduct a fire drill and will submit a copy of the fire drill log.
Coffee table located at the living room, needs safety bumpers around the table.

Licensee agreed to provide photos/copy of documents via email to LPA Miranda no later than 04/ 04/2022.

An exit interview was conducted with the above items discussed and a copy of this report was provided to Licensee Shaunaw Ricks Scalmados.
