Licensing Program Analyst ( LPA) SIlva Garibyan conducted an announced Pre-licensing inspection to observe the new facility for Valor Montessori Preparatory School . On 12/18/2020 at 11:30 am LPA Garibyan met with owner Janika Surtherland, who provided LPA a tour to observe the facility for pre licensing. Application was submitted for a Preschool license with a toddler option. All programs will be measured and considered for licensing today. Control of property was verified with a copy of the Commercial Lease Agreement. The fire department has approved the fire clearance for the capacity of 115 children. Fire Clearance granted on 11/13/2020 by Inspector Cindy Struck, LA City Fire Prevention Bureau.
LPA toured the facility inside and outside. There are six classrooms assigned for the preschool program. Toddler option is also requested with the preschool license. Toddler option classroom is separated from the pre school classrooms.

The following are the measurements:

Preschool Indoor:
Classroom #1 : 17 x 29 = 493 sq. ft /35 = 14
Classroom #2 : 19.4 x 29 = 562.6 sq. ft /35 = 16
Classroom #3 ( Library) : 29 x 17.8 = 516.2 sq. ft/35 = 14
Classroom #4: 18.3 x 28.5 = 521.55 sq. ft/35 = 14
Classroom #5: 16.8 x 29.9 = 502.32 sq. ft / 35 = 14
Classroom #6 ( MPR): 23.9 x 86.5 = 2,067 sq. ft./35 = 59

Total indoor measurement for preschool : 4,662.67 sq ft. / 35 = 133 preschool children

Preschool outdoor measurement:

Quiet Area: 80 x 150 = 12,000 sq. ft
Bench Aread: 46 x 126 = 5,796 sq. ft.
Main Preschool Playground: 85 x 126 = 10,710 sq. ft.
Garden and Chickens: 9 x 136 = 1,224 sq, ft,
Basketball and Baseball yards will be used by the preschool.

Preschool outdoor total ( not including Basketball and Baseball yards): 29,730 sq ft. / 75= 396 preschool children
Toddler Room measurements are:

Toddler Classroom : ( 18.7 x 29.8) + ( 8.3 x 17.7) + ( 8 x 4) = 736.17 sq ft / 35= 21 toddlers

Toddler outdoor measurement:

Fenced Area: 27.6 x 31 = 855.6 sq. ft / 75 = 11 toddlers
Grass Area: 50 x 55 = 2,750 sq. ft./75 = 36 toddlers

There are 3 bathrooms that is used by the preschool children consist of 6 toilets, 1 urinal, and 7 sinks
There is a sink in classroom #2, #4, and #5. .

There is a bathroom in the Toddler's classroom consist of 1 toilet, 1 sink, and changing table.

Based on the total 6 toilets and 7 sinks available for preschool children's use, the facility accommodates 90 Preschool children.

Napping Equipment: Cots and mats are available for children in care. Cots and mats are equipped with sheets. Children will nap in individual classroom and napping equipment can be properly stored for safety.

Staff/Children Files: LPA observed all CCLD forms (blank) in files. Staff files were observed with sections for personnel, qualifications and evaluations. Children files were observed with sections for admissions agreement, immunization, medication consent, and authorizations.

Lunches are not be prepared by the school. Hot lunch is catered by an independent company. Menus are posted. There is a full service kitchen on site. The kitchen is equipped with a refrigerator, freezer, stove, sink and microwave oven. The kitchen has cold and hot running water. The kitchen will be off limits to children in care. The kitchen is adjacent to Classroom #6.

Postings: LPA observed bulletin board outside the director's office for parents. Bulletin board will contain school updates and required postings by licensing (Facility License, Emergency Disaster Plan, Parents Rights Poster, Personal Rights Form)

Medication: All medication will be located in the Director's office.

LPA observed the facility to be gated all around, safe, sanitary and in good repair. Isolation area is located in Directors office and equipped with a mat and cot for ill children. Separate toilet and sink ( inside the kitchen) were observed for sick children.



The following was discussed during this inspection:

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Carrying out medical orders when the child’s physician has determined that a layperson can be trained and safely carry out the orders.
Commencing September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis, and measles. Influenza waiver was discussed.

Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: www.mandatedreporterca.com
The licensee has been informed that all employees must be associated to the facility. If the licensee fails to have proof of a fingerprint clearance or fails to associate a previously cleared individual to the facility, a civil penalty of $100.00, per day the person has been present, will be assessed. The first violation is subject to the penalty for up to five days. If there is a subsequent violation in a 12 month period, the fine will continue for up to 30 days. The "Notification of Parent's Rights" (PUB394) was discussed with the licensee and the licensee was advised that it must be posted in an area of the facility accessible to parents.

Licensee was reminded it is the licensee’s responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. Licensee was also encouraged to read the Child Care quarterly updates every season as the come out to stay informed of any changes or updates to the regulations.

The licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. The following is required to continue the processing of the application for capacity of 115 ( including 15 toddlers) children:

1) Safety gates barricading the stairs to the second floor of the empty structure in the back of the property.
2) A final review by LPA Garibyan

Technical Assistant (TA) visit was also conducted. LPA sent applicant via email the following materials: COVID-19 Self-assessment guide, COVID-19 guidance, COVID-19 Infection Prevention, and Posters for children and staff. LPA observed all posters posted.

An exit interview was conducted with applicant and a copy of this report was signed by LPA Silva Garibyan. This report will be sent via email to Applicant who agrees to sign and date the report. This report was sent via email and an electronic read receipt confirms receiving the report. The Applicant was provided with the mailing address of the El Segundo Regional office and agrees to sign and mail the original report by mail.
