On 12/07/2021 at 12:19 p.m., Deborah Lowe, Veronica Wheatly, and Margarit Sislyan, Licensing Program Analysts (LPAs) and Mary Ruiz, Licensing Program Manager (LPM), conducted an unannounced Comprehensive Case Management visit. During today’s visit LPA’s met with Peggy Nairn, Owner/Director, and toured that facility indoors and outdoors. There was 1 adult providing care and supervision to 7 children. Days and hours of operation are Monday – Sunday 4:00 am to 11:00 pm. LPAs verified the facility phone number is (818) 882-8022 (and the phone is transferred to Owner Director Peggy Nairn). Per Stipulation and Waiver; and Order the facility is on Provisional License for 1-year effective May 13, 2021.

School age children were not observed upon LPAs arrival, Peggy stated school age children were at school. At 1:46 pm 3 school age children arrived at facility and was observed under direct supervision of 1 adult. At 3:25 pm 4 more school age children arrived at facility making total school age children in attendance to be 7 children with 1 adult. Peggy's 3 dogs were observed on leases in the director office. Peggy provided LPAs with the vaccination record for the dogs.
There is no swimming pool or other bodies of water on the premises. Disinfectants cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, lose or pointed parts. Outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard were observed in classroom and entry way. Ladder observed in the walkway of the entry way. Furniture, chairs and tables stacked in a corner create a potential to fall and harm a child. Classroom pet cages were observed to be obstructing a door in the classroom from indoor to outdoor. Playground equipment is in safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Toilets and hand washing facilities are in safe and sanitary operating condition. School age children were observed to be using the same one single use restroom.

Restroom observed is not the restroom listed on the facility sketch. Floors in the facility are clean and safe. Kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Food is protected against contamination and any contaminated food is discarded. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files including children’s files in attendance and observed files ( were complete with: Identification and Emergency Information (LIC700, Child’s Preadmission Health Evaluation (LIC702), Physician Report (LIC 70, if not enrolled in elementary school), Consent for medical Treatment (LIC627), Immunizations Records (“Blue Cards”, PM286 for non -school-age children), Current Admissions Agreement with signature, Notification of Parents’ Rights (LIC995), Personal Rights (LIC613A). Children’s files do not have signed acknowledgement of being provided a copy of Stipulation and Accusation and is not available for review upon request. There are no school age children with a Restricted Health Condition. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for, influenza (current), pertussis and measles and current documentation of completed Mandated Reporter Training, Health Screening Report (TB), Personnel Record (LIC501), Teacher Qualification (LIC9095), Director Qualifications (LIC9096), Appropriate Official Transcripts, Criminal Record Statement (LIC508), Notice of Employee Rights (LIC9052), Statement Acknowledging Requirement to Report Suspected Child Abuse (LIC9108). Staff are provided 24 hours of quarterly training on applicable Tittle 22 Regulations, including Mental Health Assessment, Client Behaviors, Personal Rights, Care and Supervision, and Medication; records of staff receiving training are available for review. Training log was provided school age staff were not listed as completing training provided.

There is a Director on the premises during the day program and evening program. There is a clear written statement of the administrative responsibility and authority delegated to the child care center director and a copy of this written statement has been given to the child care center director and has been made available to the Department upon request. There are no employees working more than eight (8) hours per day. Menus are posted at least one week in advance where an authorized representative can view them. The Stipulation is not posted in a conspicuous place at the facility. The updated Facility License reflecting the Provisional License is posted.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency(y)is is/are being cited: (please see LIC 809-D for cited deficiencies). Licensee was provided a copy of their appeal rights.

A copy of this report was explained and provided to Peggy Nairn. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
