On 02/17/2022 at 8:30 am Licensing Program Analyst (LPA) Deborah Lowe met with Applicants, Arshaluys Indzhyn and Mariam Khachatryan. The purpose of the visit was a second announce pre-licensing inspection to review for a change in ownership application at the facility to ensure that health, safety and personal rights as requires by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. This is an application for a change of ownership for 16 infants, ages 0 months to 24 months. The child care center will operate Monday – Friday 7:00 a.m. – 5:30 p.m. The fire clearance was approved on 12/10/2021. Applicant has a pending applicant for a Preschool License 197494975.

LPA Lowe observed 9 infants present under direct supervision of 3 staff during this inspection. Staff present were observed to have their criminal record clearances.

The 1st pre-licensing visit was conducted on 01/28/2022. LPA Lowe is conducting a 2nd pre-licensing visit to evaluate the facility corrections needed before a license application will be considered.

  • Repair window screen in Infant room near bathroom – LPA Lowe observed the window screen to have been replaced and the new screen was observed to be in good repair.
  • Ensure crib area is separate from infant activity space – LPA observed a child safety gate separating the infant activity space from the crib area.
  • Provide thermometer for Infant refrigerator – LPA Lowe observed a thermometer in the infant refrigerator.
  • Repair metal fencing on exterior of infant playground – LPA observed the metal fencing on the exterior of the infant playground to have been repaired.
  • Ensure internal fence around infant playground is 4 feet or higher – LPA Lowe observed a new internal metal gate surrounding the infant playground. LPA Lowe measured the fence to be 4 feet in height.
  • Remove outdoor play equipment that is not age appropriate for Infants. – LPA observed the play

equipment in the infant outdoor play area to be age appropriate for children ages 0 months to 24 months.

During visit LPA Lowe reviewed facilities mass casualty plan. LPA advised facility to keep mass casualty plan available to be reviewed during future facility visits when requested.

The following items were discussed with applicants:

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.
