32 | LPA Clayton conducted a full investigation, which included facility visits, conducting interviews with the Reporting Party (complainant), staff, and parents present on the day mentioned in the complaint allegations. Based on LPA's interviews the preponderance of evidence standard has been met, and the above allegation is found to be SUBSTANTIATED.
A Type B Deficiency is cited in accordance with Title 22 California Code of Regulations and/or Health & Safety Codes see LIC 9099D).
An exit interview was conducted, the report and Appeal Rights were reviewed and provided to Director Makhaila Causey.
