Licensing Program Analyst (LPA) Maddox met with Latasha Mauldin Director, today for the purpose of conducting an unannounced Annual/Random inspection. This Pre-School program is held on the campus of ILead Elementary School in the Multi-purpose room. The hours of operation are MONDAY - FRIDAY 6:30 AM - 6:00 pm

**LPA noted age appropriate furniture, equipment, toys and materials. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in individual baskets as you enter classroom. Drinking water is available inside the classroom in the form of Igloo with disposable cups.

**The children's bathrooms are located within each classroom. The front bathroom contains 3 toilets and an open sink with 3 faucets; the rear bathroom has 2 toilets and another open sink with 3 faucets - 1 single sink inside the bathroom.

The staff restroom is in the rear classroom. LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. Toilets and sinks are functioning properly and age appropriate.
**Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Center utilizes sand under climbing structures for cushioning material and there is enough and adequate shade available. Large play equipment and structures are securely anchored. The area was observed to be free of debris. Staff bring water outside for children (igloo with disposable cups).

**LPA observed working smoke detectors, fire extinguishers, and carbon monoxide detectors in the Multi purpose room. Staff sign children in and parents sign children out; LPA observed Licensing forms posted on the Parent Board minus the License.

**Breakfast and snacks are provided by the school during the school year, in the Summer months, school operates full day and the school supplies Breakfast, Lunch, and afternoon snacks.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm - Director states there are no children present whom require IMS, but there is a plan in place.

Exit interview conducted, copy of report left with Director. There were 6 type B citations issued today as a result of this unannounced inspection, a copy of this report and citations were left with Director at the conclusion of this inspection.
