Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced annual random site inspection. LPA met with Lead Taecher Julie Gray. LPA inspected the inside and outside of the facility. LPA observed 17 school age children with 3 teachers. This facility is a Title 5 State Preschool that operates half day from 7:25 a.m. to 10:25 a.m for morning class and 11:25 am to 2:25pm for afternoon class, Monday through Friday. This facility provides breakfast for morning class and lunch for afternoon class. Food services for both classes are provided through school cafeteria. Menus were posted and visible.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual cubbies with children’s name labeled for children's belongings. An isolation area was inspected (Staff office).

This is a half day program (children do not nap). Age appropriate sinks and toilets were inspected for availability and good repair. Toilet paper, paper towels were observed inside the restroom. First Aid supplies, smoke detectors, carbon monoxide and fire extinguishers were observed. Trash cans with tight lids were observed.

Sign in and out sheets were reviewed, and complete with full legal signature. The facility roster was up to date and all staff have been fingerprint association to the designated license number. Director and teachers are currently certified in Pediatric CPR/First Aid valid 11/21/2019 through 11/21/2020 . Fire and disaster drills are conducted , last drill was completed on 03/05/2020. Children's records and staff records were reviewed and complete.

There were no bodies of water observed in the playground area. The outdoor playground was inspected and was observed to be free of hazards, loose and sharp parts. There are two water fountains available to children during outdoor play. The playground was observed to be properly gated all around. Equipment was inspected for safety, cushioning material, good repair and age appropriateness.

The parent board was reviewed and has all of the required forms posted accessible to parents.

The following Incidental Medical Services (IMS) were discussed.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, child care providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Fingerprint clearances and transfers: Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state licensed facility to another using form LIC 9184

Per the Title 22 regulations, the above facility was found to be operating in substantial compliance.

A copy of this report must be made available to the public for 3 years.
