On July 6, 2023, Licensing Program Analyst (LPA) Annelise Villa met with Director Keelan Pratt to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) that occurred on July 6, 2023. The unusual incident report was regarding an incident involving child 1 (C1). Upon arrival, there were 74 preschool age children, and 17 staff providing supervision at the facility.

During this inspection, confidential interviews were conducted with staff. In addition, LPA reviewed staff files and obtained documentation related to the case management. Furthermore, LPA also completed a safety inspection of the Center.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Keelan Pratt.
