On 6/12/2019 at 8:40 a.m., Licensing Program Analyst (LPA) Brianna Reynoso met with above facility's Director, Tamra Lewis. LPA was at the facility to conduct an unannounced prelicensing inspection as the facility had submitted a capacity increase and change of corporation application to the Palmdale Regional Office.

The facility's hours of operation are Monday through Friday from 6:30 a.m. to 6:00 p.m. They provide snacks and optional hot meals which are purchased and prepared by the facility. The facility does not transport any preschool children in care.

During this inspection, LPA measured the indoor and outdoor activity spaces that will be utilized for preschool children. The center has seven classrooms for the preschool children in care. Per the Director, Room 7 and 8 are are the Beginners classrooms, Room 2 and 6 are the Intermediate classrooms, and Room 1, 3, and 4 are the Pre-K classrooms. The facility is currently not using Room 5 for children, as it is being used for the church's ministry. The measurements for each classroom are as follows:

Room 1: 653 square feet / 35 = 18 children


Room 2: 751 square feet / 35 = 21 children
Room 3: 760 square feet / 35 = 21 children
Room 4: 824 square feet / 35 = 23 children
Room 5: 783 square feet / 35 = 22 children
Room 6: 761 square feet / 35 = 21 children
Room 7: 767 square feet / 35 = 21 children
Room 8: 729 square feet / 35 = 20 children

The measurements for all classrooms combined totals to 167 children.

During today's inspection LPA also measured the outdoor activity spaces for the preschool children. The facility has a Beginners through Pre-K yard, and a Beginners Only yard. The measurements for each play yard are as follows:

Beginners through Pre-K Yard: 12,467 square feet/ 75 = 166 children


Beginners Only Yard: 524 square feet / 75 = 6 children

The measurements taken for both yards combined totals to 172 children.

Based on the indoor and outdoor activity space measurements taken, a capacity increase request for 84 children will be granted.

LPA observed the outdoor play areas to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, conditions and age appropriateness. Climbing structures, slides, and play equipment were found to be securely anchored with resilient cushioning material underneath and around the perimeter. Children will have access to water through water jugs and disposable cups during outdoor activities. The playgrounds are fenced all around, and no bodies of water were observed in the outdoor play areas. The playground areas have a shaded rest areas for the children.

Due to the square footage of the Beginners Only yard, LPA reminded Director the facility may only have six children on that yard at one given time.

LPA observed and inspected three restrooms for the children in care. One restroom was located inside of Room 8, and the other two gender separated restrooms were located inside of the church lobby. Church members have a separate restroom from the children in care, and the facility has placed signs above each restroom indicating they are to be used by day care children only. Each restroom had toilet paper, hand soap, and paper towels accessible to children in care. LPA observed a total of nine toilets, eight sinks, and two urinals, all of which were in operable condition.

Furniture and equipment were inspected for operable conditions and age appropriateness. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual storage for children's belongings. Kitchen area located inside of the church, was adequately equipped, clean, and free from hazards. The kitchen has a sink, stove, oven, refrigerator, and freezer. Food was properly stored, labeled, and within expiration date. Menu was posted, dated, and accessible for parent’s review. The facility's food allergy list is posted inside of the kitchen and in each classroom. LPA observed a parent board located inside of the preschool office above the fish tank.

First Aid supplies, dual smoke and carbon monoxide detectors, and fire extinguishers were observed and in operable condition. All fire extinguishers (2A10BC) had a service tag attached indicating they were last serviced on 6/4/2019. Trash cans with tight lids were observed. LPA observed a First Aid Kit located inside of each classroom. The Director has a current CPR and Pediatric First Aid certification, which expires on 7/2020. All other over the counter medications are stored inside of the preschool office, in a locked cabinet.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required. The following was discussed with the Director:
Smoking is prohibited on the premises of a child care center; Infant walkers, baby jumpers, exersaucers, baby rockers and any other item that falls into that category are prohibited; Earthquake safety and necessity of drills are to be conducted every 6 months and logged; Required forms for children’s files, facility files, staff files, and posting requirements; Fingerprint clearances and the transfer process.

Based on today's inspection, the facility was in compliance per Title 22 regulations. The facility's capacity increase request has been granted, and they are ready for licensure.

An exit interview was conducted, a copy of this report, and notice of site visit were provided to Director, Tamra Lewis.
