On 8/06/2019 at 11:35 a.m., Licensing Program Analysts (LPAs) Brianna Reynoso and Isabel Ortega met with above facility's Director, Francisco Rodriguez. LPAs were at the facility to conduct an announced prelicensing inspection as the facility had submitted a new application for a School Age Child Care Center.

The facility's hours of operation will be Monday through Friday from 6:30 a.m. to 7:30 a.m. After School: Monday, Tuesday, Thursday and Friday: 2:32 p.m. through 6:15 p.m. and Wed: 12:47 p.m through 6:15 p.m.. They will provide morning and afternoon snacks, which will be purchased and prepared in the Rise classroom 103 by the facility. The facility will not be transporting school age children.

During this inspection, LPAs measured the indoor activity space that will be utilized by the facility. The facility will be using one classroom known as the Rise Room 102 and room 103 for the children in care. The measurements for each classroom are as follows:

Classroom Rise 102: 634.55 square feet/35 = 17 children


Classroom Rise 103: 620.61 square feet/35 = 17 children

A fire clearance request for capacity of 40 school age children was granted on 4/4/2019 however, based on today's measurements the square footage is sufficient enough for 34 school age children.

During today's inspection, the Director contacted the Superintendent from the Castaic School District, Steve Doyle. The facility has informed LPAs that in accordance with the Health and Safety Code 1596.806(2)(B), they will be providing the department with a certification statement from the Superintendent. The statement will authorize the occupancy for each classroom the Castaic School District is allowing the facility to utilize for their school age program.

The facility will be using the playground on the NorthLake Hills Elementary School site. The facility is exempt from square-footage requirements for outdoor activity space, and toilet requirements as they are operating in accordance with Health and Safety Code Section 1596.806.

LPAs observed the outdoor play areas to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, conditions and age appropriateness. Climbing structures, baseball diamond, handball wall, tetter ball, basket ball rings and play equipment were found to be securely anchored with resilient cushioning material underneath and around the perimeter. Children will have access to water through water fountains during outdoor activities. The playground was fenced all around, and no bodies of water were observed. LPAs also observed shaded rest areas for the children in care.

LPAs observed and inspected two gender separate restrooms for the children in care, which were located outdoors on the elementary school site. Restrooms had toilet paper, hand soap, and paper towels accessible to children in care. LPAs observed a total of 19 sinks, sixteen toilets, and six urinals which all were in operating conditions.

During today's inspection, the Director provided LPAs with a indoor activity space waiver, as they would like to use two of the restrooms located in the preschool classroom next door. The reason for utilizing these restrooms would be for emergency purposes and as an isolation restroom for children who are ill, as the primary restrooms for the school age program are all located on the elementary school site near the play yard.

Furniture and equipment were inspected for operable conditions and age appropriateness. Telephone service, heating, lighting and ventilation were evaluated. LPAs observed storage space inside of each classroom. Snacks will be stored inside of the Rise Room 103, and the parent board will be located inside of this classroom, to the right of the entrance of the classroom.

LPAs observed a First Aid Kit located inside of each classroom. The fire extinguishers were observed and in operable condition. All fire extinguishers (2A10BC) had a service tag attached indicating they were last serviced on 7/17/19. Trash cans with tight lids were observed. The Director has a current CPR and Pediatric First Aid certification, which expires on 2/2020.

The facility has a current annual certification for the fire alarm system, which was conducted by Fire Ace Department on 2019. The certification is proof that the fire alarm system as well as the smoke system is in operating conditions.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com

The following was discussed with the Director:


Smoking is prohibited on the premises of a child care center; Infant walkers, baby jumpers, exersaucers, baby rockers and any other item that falls into that category are prohibited; Earthquake safety and necessity of drills are to be conducted every 6 months and logged; Required forms for children’s files, facility files, staff files, and posting requirements; Fingerprint clearances and the transfer process.

Based on today's inspection, a license for a School Age Child Care Center will be granted once the following have been completed: The facility submits the certification statement from the Castaic School District's Superintendent, as well as a copy of the Director's Mandated Reporter Certificate (AB1207), as the one on file is for school personnel.

An exit interview was conducted and a copy of this report was provided to Director, Francisco Rodriguez.
