On November 10, 2020, Licensing Program Analyst (LPA) Tsutaoka conducted an announced Case Management - COVID-19 tele-inspection. The inspection was conducted using Facetime as a safety precaution due to COVID-19. LPA called Director Yesenia Canchola who guided LPA on a virtual tour of the facility. LPA informed the Director of the purpose of the visit.

Staff 1 (S1) last attended the center on October 9, 2020. S1 started to have symptoms after she left work around 6:00PM. She went to go get tested on October 11, 2020, and got results back Monday, October 12, 2020. S1 self-quarantined herself until Monday, October 26, 2020. Director was informed S1 tested positive on October 13, 2020 and informed parents to monitor their children due to the staff testing positive.

On October 16, 2020, Parent 1 (P1) informed Director she had tested positive for COVID-19, so she kept Child 1 (C1) home as a precaution and went to go get him tested. On October 17, 2020, C1's test results came back positive for COVID-19. Director decided to close the facility from October 19, 2020 and reopen on October 26, 2020 because there were positive test results in two separate classrooms. C1 was cleared to come back October 26, 2020, but C1 has not returned to facility because P1 has pneumonia. On October 16, 2020, Director reported the positive exposure to Licensing.

On October 18, 2020, Director disseminated a message to all parents the facility will close from October 19, 2020 through October 23, 2020. On October 18, Parent 2 (P2) notified Director she tested positive and is going to take Child 2 (C2) to go get tested. C2 tested positive for COVID-19 on October 23. C2 was cleared to return to the day care on November 2, 2020. The toddler classroom which C1 and C2 attend was deep cleaned by a cleaning crew.

Director has all staff go get tested for COVID-19 every two weeks. Staff 2 (S2) went to go get tested on October 31, 2020 and received positive test result on November 2, 2020. S2 got her test results via text message while at the facility on November 2, 2020 and left the facility as soon as she received it. Per Department of Public Health (DPH) recommendation, Director shut down that cohort and had the two children that could have possibly been exposed self-quarantine. The cohort (children and teacher) are cleared to come back to facility November 16. The classroom was deep cleaned on November 2, 2020, November 4, 2020, and November 6, 2020 by a professional sanitizing crew and the entire center was sanitized as well.

The facility has a cleaning schedule where all teachers clean the frequently touched surfaces in their classrooms with the Sam's Club brand sanitizer.

Director did not submit an Unusual Incident Report (LIC624) to licensing regarding the staff and children who had tested positive for COVID-19 on October 18, 2020, October 23, 2020, and October 31, 2020 within 7 days of the incident.

A Type B deficiency was cited during this inspection.

Director was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. A copy of this licensing report (LIC809) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed.

An exit Interview was conducted, a copy of this Report, Appeal Rights, and a Notice of Site visit was provided to the Director.
