Licensing Program Analyst (LPA's) Neal met with the Administrator, Sandra Denkovski and Director Anais Femat for the purpose of conducting a pre-licensing inspection of the School Age center. During this Pre-Licensing inspection, LPA toured the facility according to facility sketch. Center is located at the Acton- Agua Dulce Unified School District. Facility will service children of ages 4.9 to 13 years old. Currently, the center consists of 1 modular (Room #122). Applicant is considering adding an additional modular (Room #123) and will notify the department when board approval documents have been submitted, the 2nd modular has furniture/supplies and is ready for use. Center will operate Monday through Friday 6:00am to 6:30pm.

LPA observed age appropriate furniture. The classroom was observed to be clean, safe and free of any health and safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. There will be a space for children's belongings. Medications will be stored in a lock box. Tables and chairs were observed to be in good condition, free of sharp, loose or pointed parts. There are moveable bins for solid waste kept in good repair.

Per Title 22, Section 101538.2(c)(1) and Health and Safety Code Section 1596.806 regulations, activity space is located on a functioning public school site, therefore, the site is exempt from square-footage, toilet, isolation space, outdoor activity space and fencing requirements for child care centers.
Applicant was reminded that capacity and ratio must comply with specified regulations.

LPA inspected the playground area. There is a blacktop area, grass area and large anchored play structures with sand underneath for active play. One play structure is designated for age groups 5 to 12 years old per manufacturers label. Applicant will ensure that children younger that 5 years old utilize other play areas or the adjacent play area that is age appropriate.
Staff restroom is located beside the front office. Children will use the bathrooms located across the playground to the right. There are separate bathrooms for boys and girls (boys=2 urinals, 2 toilet stalls, 2 sinks; girls = 3 toilet stalls, 2 sinks).
The facility serves afternoon snack. The facility's snack menu will be posted on parent board at the entrance in the room. Fire extinguisher, smoke detector and carbon monoxide detector were all observed. Applicant was informed to conduct and maintain Fire/Earthquake drill logs. Required forms will be posted where parents can see. Director is certified in Pediatric CPR and First Aid (exp. 08/2020). Mandated Reporter training was completed. Applicant was reminded that all staff/volunteers will need to complete the Mandated Reporter training every 2 years. The facility has sign in/sign out sheets.

This facility will provide Incidental Medical Services – IMS as specified in their Parent Handbook. Any licensing report documenting a Type A citation shall remain posted for 30 days along with the Notice of Site Visit. In addition; all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt.

**Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed child care facility are aware of situations that present the greatest danger to children.

Applicant was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624.
Applicant was reminded to access the licensing website at ccld.ca.gov to obtain information about the most recent regulatory changes and the Quarterly Updates.
Transportation will be utilized in the form of the school buses and there is a designated meeting point at the gate. There will be a sign in/out and check in/out procedure in place for children to ensure safety and supervision.
Fire inspection was granted on 12/10/2019 for Room #122. Room #123 is pending confirmation of inspection under the school district.
License will be issued upon the following corrections:
1. Some form of Drinking Water will be installed.
2. Toys, equipment and children's supplies will be added.
3. Required licensing documents will be posted on parent board.
4. Applicant will notify LPA when Room #123 is ready for inspection to be utilized as discussed (board approval, updated facility sketch, supplies and confirmation of fire safety inspection through school).
5. Proof of Required immunizations will be submitted for Administrator and Director.
Once corrections have been verified the application for the school age center will be submitted for approval. Applicant is advised that all corrections are due within 30 days or the application may be withdrawn.
