Licensing Program Analyst (LPA) Maddox met with Parag -Laddha, Samantha Scalise, Director, Karen Okicich, Regional Manager, and Stephanie Retherford, Regional Vice President today for the purpose of conducting a Pre-Licensing inspection for this School age component. Owner has also submitted applications for a Pre-School Component (X197750097) and Infant/Toddler component (X197750096), a physical separation must be maintained for each individual component. This center is located within a strip mall. Center will utilize the "Core" Electronic system to for parents to sign in and out. In addition, parents will also sign in and out manually. Parents will use a Key Fob to enter the building, once inside the center, LPA observed a sanitation station. The hours of operation: MON - FRI FROM 6:30 am to 6:30 pm. **The Parent Board (located in the main entrance area).

**Classrooms were equipped with age appropriate furniture, equipment, toys and materials. The classrooms were observed to be clean and safe and free of any Health or safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall as you enter classrooms. Drinking water is available inside the classroom in the form of a water cooler and disposable cups. The Pre-K classroom is equipped with it's own bathroom (1 toilet and 1 sink).

Owner is aware that prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state licensed facility to another using form LIC 9182.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm - Center has an IMS plan in place.

Staff are aware that prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state licensed facility to another using form LIC 9182.

LPA informed staff to check with their local Health Departments regarding Cohort sizes, and facial coverings.



There was no outside shade in place on the play yard for School age children, LPA requesting applicant forward a picture of shade structure once it's been ordered and in place.
Capacity will be approved for requested capacity of 120 Pre-School Children upon receipt of correction. Exit Interview Conducted.

The Pre-K children's bathroom is located within the classroom, the bathroom has 1 toilet and 1 sink.
Capacity with toilets and sinks: 15

Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Staff will bring water outside for children. LPA observed the fencing surrounding the play area to be at least 6 ft tall to protect children and to keep them in the outdoor activity area, the gate was locked during this inspection and is equipped with an alarm. There are water fountains on the play yard that will not be used right now due to the COVID-19 pandemic.

PS Yard: 3630/75 = 48

**Food preparation area/Kitchen was inspected for safety, cleanliness, proper equipment & protection against contamination and storage. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children

LPA observed a fully stocked first aid kit; fully charged fire extinguishers; carbon monoxide detectors throughout the center. Fire Drill/Earthquake log is posted. Director and Managers are designated to dispense medications if needed.

LPA has verified staff have taken the Mandated Reporter Training; Staff have required Immunization's: and Lead Poisoning training.
