Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management Incident inspection. This inspection is regarding an incident that took place on February 7, 2022. Licensing Program Analyst met with Assistant Site Supervisor Amber Pimentel who providing assistance for the inspection.

During the course of the inspection, from 2:50pm to 4:00pm LPA conducted interviews (with Staff #1, Staff #2, and Director Pimental). Note: The incident is regarding a disclosure to the facility indicating child #1 sustained an injury requiring medical.

LPA also reviewed internal incident reports provided by the facility. There was no disclosures indicating that a Title 22 violation occurred regarding the incident. Note: The child is currently at the childcare facility with no medical restrictions. On February 7, 2022 Department and a Children's Family Services Social Worker and two Long Beach Police Department Detectives visited the facility and interviewed only Director Pimentel. Staff #1 and Staff #2 were not interviewed.

LPA will request other information and/or reports from the two outside agencies mentioned. Based on interviews and review of internal reports there are no violations of Title 22 requirements at this time.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Assistant Site Supervisor Amber Pimentel.
