Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Required 1 year inspection. LPA met with Assistant Site Supervisor Amber Pimental who guided analyst on tour of the facility. This is an infant program which consists of two classrooms serving children from zero (0) to two (2) years old. Facility operation hours are Monday through Friday from 7:00 AM to 6:00 PM.

At 9:55am All areas identified on the Facility Sketch were inspected. LPA observed the following staff were caring for children during the inspection: Infant Room: One teachers, One aide with five infants. Toddler Room: Two teachers, three aides with 17 infants (observed to be within the required Teacher - infant ratio). All furniture and equipment was inspected for age appropriateness and good repair. LPA also observed adequate telephone service, heating, lighting and ventilation. Storage for infants’ belongings and napping equipment/cribs was inspected and found to meet all regulatory requirements at this time. The changing table has raised sides at least three inches high and is within an arm’s reach of a hand washing sink. LPA observed toys and play equipment to be clean. There is drinking water available indoors and outdoors.

LPA observed a first aid kit kept kept in the classroom. Carbon monoxide detectors and smoke detectors are present in the facility. The facilities 2A10BC Fire extinguisher gauges are in the green and due for service October 2023. Storage cabinets in the classrooms are locked and hazardous items including medicine and cleaning compounds are stored inaccessible to children.



At this time, the office is used as an isolation area. There are portable cots and changing mats for infants/ to rest and be changed if needed. The facility provides breakfast, lunch and snack. Some parents provide breast milk for infants. The parents bring infant sheets and staff wash everyday. Older infants sheets are sent home each week to be laundered. Food preparation areas were toured for safety, cleanliness and proper equipment. Bottles, dishes, and containers are labeled with infants’ names in the refrigerator. LPA informed Director to ensure that names are always legible on bottles and cups to all staff that may assist with infants.

Outdoor equipment was inspected for safety, cushioning material, good repair, and age appropriateness. LPA observed the outdoor area to have required shade, available drinking water and adequate fencing (at least five feet high). LPA observed no out of the ordinary hazards and or bodies of water outdoors in the outdoor play area. LPA Birks advised Site Supervisor Pimental to constantly remind staff that Infants need to be within the direct care and supervision, including visual supervision of staff at all times.

Care and supervision was evaluated to determine if the basic needs of infants are met and appropriate. Infant Needs and Service Assessments are done quarterly. LPA observed all children were signed in by Parents and/or Authorized Representatives digitally. At approximately 11:00am LPA reviewed ten children's files to ensure completeness and to ensure files contain proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights, Physician's Reports, LIC 9227 Infant Sleep Form and Safe Sleep log.

At approximately 11:45am Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination. LPA observed at least one staff member to have current CPR (Expiration Dec 2024). All staff have the current required Mandated Reporter training on file.


Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following was discussed with the Site Supervisor : Recently Approved Safe Sleep Regulations in Effect (PIN 20-24-CCP). LPA also continued to advised staff to sleep infants on their backs and check on them at least every 15 minutes for issues such as labored breathing, flushed skin color, increased body temperature and restlessness. The 15 minute checks must be documented.

When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Mandatory forms for the children’s files and staff files, requirements for fire drills, earthquake drills (last drill October 2022) and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Director was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. LPA advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.


LPA advised Assistant Director that all adults 18 years of age and older providing Care & Supervision and having a continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. LPA informed Director to log onto web site www.ccld.ca.gov to access forms, regulations and quarterly updates. Records for all children and staff must be maintained for three (3) years after separation from the facility. Report Unusual Incidents and/or injuries to the parent/guardian immediately and to Child Care Licensing within 24 hours and/or the next business day. Visit www.shotsforschool.org for immunization information. All care for children require supervision at all times. If away from the facility, staff must be with children at all times (especially when outdoors). There were no violations of Title 22 requirements at this time.

The Notice of Site Visit (LIC 9213 –must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
