On February 9, 2024, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required 3-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPA entered facility through the Sparks Elementary School’s front office and was instructed to walk over to the east side of the campus where LPA met with staff who allowed LPA to enter through a chain link fence into the Head Start preschool program Room #26. LPA met with On-Site Supervisor/Teacher, Enda Wang and LPA took a census of 17 children and 2 staff in care. The program operates as a Head Start preschool program. Staff called and Program Manager, Monica Javis arrived at 9:30 am and Child Development Supervisor, Kathy Jarvey arrived at 10:10 am, who are located off-site at their main office. Program Manager and Child Development Supervisor arrived to facilitate the visit.

Hours of operation: The days and hours of operation for the Head Start Preschool: Monday-Friday, 8:00 A.M.- 2:00 P.M.

Location: Facility is located on the east side of the main office of Sparks elementary school. The portable and the playground are surrounded by a chain-link fence, separating it from the rest of the elementary school. There is a portable which is not licensed next door that is a resource room. The portable is one large classroom with three restrooms: two for the children, one for staff, a kitchen, and a staff office. The chain-link fence which surrounds the portable are kept locked during operating hours and are only unlocked when children are being dropped off and picked up. The children enter through one door and staff and children have access to an emergency exit door on the other side of the portable. To gain access to the school there is a side gate for parents to drop-off and pick-up children. There is a playground which has a structure that is appropriate for ages 2 -12 years of age. There are toys and gross motor equipment kept inside a storage unit.

All posting requirements were observed inside the portable, on the Parent Board which is located on the wall adjacent to the door through which the children will enter. LPA viewed a copy of the monthly menu, which is posted inside. Parents also have access to the lunch and snack menu on class Dojo, as well as the District website. All food is provided by the School District. Children are served Breakfast at 8:00 am, lunch at 11:00 am and P.M. snack at 1:00 pm.

Portable has a kitchen area which contains a full-size refrigerator. There is currently one (1) child who requires medication and is kept labeled "Medication" and locked in a cabinet in the kitchen with a padlock. LPA observed medication which was not expired and required paperwork was stored with the medication. LPA observed and inventoried First Aid Kits, which is kept in the kitchen cabinet which is labeled with a sign. The cleaning supplies are kept in the cabinet under the kitchen sink, which is locked with a chain and padlock.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. There is a carbon monoxide and smoke detectors that are connected to the elementary school and are tested by the school district through-out the year. The last annual maintenance on the Fire Extinguisher was on August 8, 2023. Last Fire Drill was documented and conducted January 2024.

Inside the portable, there are two (2) children's restrooms, which has one (1) child-size toilet and (one) 1 child-size sink located inside each bathroom near the kitchen. According to staff children bring water bottles from home for the use of drinking water and refills are provided using Brita Water Pitcher and disposable cups. The filtered water pitcher is filled from the kitchen sink. The adult restroom is located inside the portable and is used by staff only.

There is napping equipment kept in the Head Start program. Children nap and bring napping items from

home on Monday and is sent home on Friday to be laundered. Napping cots are labeled with each child’s name and cots are sanitized each day.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

Child Development Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Staff is fingerprint cleared by the School District

Child Development Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days.
