At 12/03/2021, Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced Required – 1 Year Inspection at 9:35 AM. LPA conducted a COVID-19 screening call prior to entering facility. Facility is currently licensed for a capacity of 54 but have 27 enrolled. Facility conducted COVID-19 temperature check prior to entry of facility. Licensing staff met with Cassandra Gallardo, Toddler Lead Teacher(S1). S1 states that Director Alchai Jones is on site 3 days a week and is at other sites the other days. LPA spoke to Director over the phone and confirmed S1's statement. Director also states they have recently hired a new Director to be on site but have designated responsibility to S1 until then. The program currently operates Monday thru Friday from 6 AM to 6 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors.

At the initial start of the site visit, there is a total census of 16 children and a total census of eight teachers present. The facility is made up of 3 Toddler Rooms, 1 Preschool Room, 2 Bathrooms, Kitchen, and 2 outdoor play yards. Due to low enrollment, the facility is currently just using the 2 Toddler Rooms (92MB-TB & 92MB-3C) with a shared Bathroom and 1 Preschool Room with it's own Bathroom. At 9:50 AM, LPA observed both Toddler Rooms, which are next to each other separated by a room divider, and observed furniture and equipment for age appropriateness and good repair. LPA observed small wooden tables and chairs through out the rooms, shelves filled with manipulative toys, blocks, dramatic play toys, art materials, and quiet reading area in both rooms. LPA observed water jugs in each classroom used for drinking water. S1 states that First Aid kit is red backpack hanging in each classroom and travels with them to outside play yard. At 9:54 AM, LPA observed red backpack hanging in Room 92MB-3C. LPA observed two toddlers with S2 and S3 in Room 92MB-3C. At 9:58 AM, LPA observed Toddler Bathroom which is shared by both rooms to have two toilets and two sinks, both operable at time of inspection. LPA observed adequate handwashing supplies, floor free of hazards with anti-slip rugs. At 10:05 AM, LPA observed Preschool Room, which is located down hallway to the opposite side of Toddler Rooms, and observed furniture equipment for age appropriateness and good repair. quiet reading area. LPA observed twelve children in Preschool Room with 3 staff members (S4, S5, and S6) heading out for outdoor play. At 10:06 AM, LPA observed water jugs in classroom for drinking water and a red backpack hanging for First Aid Kit. At 10:09 AM, LPA observed Preschool bathroom to have two toilets and two sinks, which were operable at time of inspection. LPA observed adequate handwashing supplies, handwashing posters posted, floor to be free of hazards with non-slip rugs. At 10:10 AM, LPA observed small Terminix traps behind toilets at time of inspection. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings in all classrooms. Mats/cots were inspected, facility uses standard blue cots for napping. Per S1, linens are provided by the facility and washed by staff weekly, LPA observed washer/dryer in hallway between classroom that is used for washing linens.

Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced on 07/12/2021. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 10:03 AM, LPA observed food preparation area and was toured for safety, cleanliness and proper equipment. LPA observed counters to be clean and free of clutter, refrigerator to be of proper temperature meeting Title 22 requirements. Per S1, facility serves Breakfast, Lunch, and Snack.

Outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. At 10:00 AM, LPA observed Toddler Yard, where two children and two staff (S7, and S8) were. LPA observed two structures with cushioning below to absorb fall. LPA observed water jug available for drinking water, and red backpack on table used for First Aid supplies, block area with child appropriate tables and chairs, and a bike track with five bicycles all in good repair at time of inspection. At 10:11 AM, LPA observed Preschool Yard, where twelve children and three staff members were. LPA observed large play structure with three slides and climbing apparatus with cushioning below play area to absorb fall. LPA observed adequate shaded area with child size tables and chairs and toys and art materials available for children to be free of hazards at time of inspection. LPA observed water jugs available for children for drinking water and red backpack for First Aid supplies. LPA Chicote advised S1 that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for adults. CPR card expires in 03/18/2023 for Staff 6(S6). Inspection of required forms made.

Incidental Medical Services:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following were discussed with the Facility Representative:
Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.

The S1 was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/03/2021. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
