On 04/05/2022 at 9:00 am, Licensing Program Analyst’s (LPA’s) Lilli Babcock and Roxana Lopez conducted an unannounced required inspection. A risk assessment was conducted prior to entering the facility. LPA’s met with Director, Cindy O’Neill, who guided LPA’s on a tour of the facility. This is a preschool age program which is operated by Rio Hondo Community College District and is located on the premises of Rio Hondo College. Facility is currently licensed for a capacity of 120 children and operates Monday – Friday from 7:00 AM – 5:00 PM. Per the Director there are 29 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: Room 1: Staff #4 and Staff #5 with 5 children; Outdoors: Staff #1, Staff #2 and Staff #3 with 15 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

LPA’s observed the facility to be clean and sanitary. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. At 9:57 am LPA’s observed paint is chipping from vertical pipe attached to outside wall in courtyard, located outside Classroom #2. Stucco next to pipe in the same area has crumbled and there is a hole in the stucco near the concrete ground. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Napping equipment (mats/cots) and bedding were inspected for good condition, appropriate storage, and cleanliness. The facility washes the sheets at the facility weekly. Storage for children's belongings were inspected. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, a room in the office is used as an isolation area. A bathroom for use by ill children is also in this area. Medications were for Child #1 and included one Epinephrine Auto Injector Pen, which expired in 5/21 and Children's Allergy Medication, which expired on 2/4/22. Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch and PM snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. There is drinking water available in all indoor classrooms and children use water bottles brought from home while outdoors. Water bottles can be refilled using outdoor drinking fountain. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children.

At 9:40 am LPA’s observed a fire extinguisher. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 12/22/2021, as indicated on service tag. At 9:44 am smoke and carbon monoxide detectors were tested and are operable.

The facility has 2 yards that are fully fenced. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.



Facility uses a digital sign in/sign out procedure. LPA’s observed all 20 children present to be signed in on this day. Last fire drill was conducted on 2/17/22. Staff Records were reviewed. Staff #2 and Staff #5 did not have current Mandated Reporter Training Certificates in their file. A sampling of 10 Children’s Record Files were reviewed and all 10 were complete. Criminal Record Clearance for adults and verification of CPR/First Aid were reviewed.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. At 10:20 am, LPA’s observed expired medications locked in the medication drawer in Room #2. Medications were for Child #1 and included one Epinephrine Auto Injector Pen, which expired in 5/21 and Children's Allergy Medication, which expired on 2/4/22. LPA's observed an email from Director to mother on 2/2/21, in which Director notified mother medications were expired and requesting new medications for Child #1. LPA's also heard Director call mother to request updated medications on 4/5/22.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
