This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. After a complete inspection of the facility, the following deficiencies were observed according to California Code of Regulations Title 22 Division 12 during today's visit as indicated on the 809D
The licensee shall require each recipient (Parent/guardian) of a licensing report documenting a
Type A citation resulting from a compliant investigation and any licensing document pertaining to a conference, and any summary of an accusation indicating the Department’s intent to revoke a license, to sign LIC 9224 form, indicating that he or she has received the documents and the date they were received. The licensee shall keep verification of receipt in each child’s file.
- Upon arrival analysts observed an infant child strapped in a car seat in the office. Child was removed from the car seat by the parent/staff member and taken to a separate area on site. Per director the child had arrived just before the analysts. The child was picked up from the facility approximately one hour after LPAs arrival.
- Rugs in the two classroom were in need of cleaning.
- Sign in sheets were missing six children.
- Mandated reporter training was not completed by all staff.
- Vaccination records were not available for all staff.
