Licensing Program Analyst (LPA) Anomeh Eivazian met with Sonia Kazadjian, Director for the purpose of conducting a case management inspection for a capacity decrease for the preschool license and add a toddler option program. This is a preschool program which consists of 5 classrooms. This facility is located on Armenian Sisters Academy Elementary School property. There is also a kindergarten program on the premises which is located on PreK Blue A, Blue B, and Orange classrooms building. Affidavit for Kindergarten program on file. Facility operation hours are from Monday to Friday from 8:00 AM to 5:30 PM. They are adding a toddler option program which consists of one toddler classroom.

Areas measured were all rooms that will be used for the preschool and toddler option classrooms. Preschool classrooms, PreK Blue A & B (4-5 year old), Orange A & B (3-4 year old), Yellow (2-3) and one toddler classroom. Also measured were the outside play areas which consists of 3 separate playground areas for the preschool and 1 play ground for toddler option.

Parents enter the facility through the main gate for both programs. Each program has its own sign in sheet in each classroom where parents will sign in and out every day.

Building/Rooms are centrally heated and air conditioned. Overhead lights as well as natural light is used. Children have their own cubbies to store their belongings.

TODDLER PROGRAM (in addition to general compliance): Toddler changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. Toys were observed to be clean and safe. Indoor activity space for this toddler program is physically separate from the preschool components at this facility, and meets the requirements of 101216.6(2).

Outdoor activity space for this toddler program is physically separate from the preschool components at this facility.

First Aid supplies were inventoried. Fire extinguishers have been serviced in 05/31/19. Per Director no bodies of water, pets, and firearms or weapons on the premises. The facility does not transport children.

LPA advised the licensee to access forms, regulations and quarterly updates, providers information notices (PINs) on the Child Care Licensing website at: www.ccld.ca.gov.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.



Medication: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. Per Director, currently they do not have any child(ren) to administer IMS. Plan to Provider IMS was submitted on 08/11/17.

There were no deficiencies cited during today’s inspection.

Per director she will submit an updated application to request for capacity of 100 preschoolers and 10 toddlers.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Sonia Kazandjian, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. A copy of this report and all other Licensing reports must be made available to the public for 3 years.

Furniture and equipment was inspected for age appropriateness and good repair. Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Friday and returned on Monday. The Director's office will be used by ill children. There is a napping cot available for an ill child. The staff restroom will used by ill children

Age appropriate sinks and toilets were inspected for availability, good repair, toilet paper, area safety and sanitation. There is one toilet and two sinks in Toddler classroom. Preschool Blue A and B and Orange A classrooms share bathrooms with Kindergarteners, waiver on file. Preschool Yellow and Orange B classrooms use the same bathrooms which are located inside the Orange B classroom. There will be supervision in the rest-rooms for both the preschoolers and the toddlers.

Snack menus were reviewed. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides AM and PM snack, and lunch. Lunch is being provided by an outside caterer. There are drinking water jugs and water fountains available in classrooms and there were drinking water fountains that were observed outdoors. Containers used to discard food have tight fitting lids. Cleaning compounds are inaccessible to the children.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are 3 outdoor preschool playgrounds (preschool back playground, preschool upper playground, and preschool sand/bike area), and one toddler outdoor playarea which is located between preschool back playground and preschool sand/bike area. All playgrounds are fenced and gated for separation of programs.

There is also cemented area for children to ride their bikes. Outdoor play equipment was observed to be in good repair. There are two stand up sand boxes on preschool playground which have leads and per director after each outdoor activity session teachers close the sandboxes and raked prior to use the sandbox.
