13 | Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection on 06/16/22. LPA arrived at the facility at 9:20am. LPA met with Director Jessy Ogbuefi, for the purpose of delivering the findings for the above allegations. Licensee gave LPA a tour of the facility at 9:30am. LPA observed 10 children with 4 staff members present during the inspection.
During the investigation, interviews were conducted with Director, staff, children, and parents. Receipts were reviewed. On 03/29/2022, during the 10-day inspection, LPA observed uneven pavement underneath the outdoor play structure located in the children’s play yard posing a tripping hazard. There was a closed bathroom stall in the children’s restroom due to toilet being inoperable. In the kitchen, LPA observe paint chipping above the refrigerator where food is prepared for children in care. The preponderance of evidence standard has been met, therefore, the above allegation is found to be substantiated. California Code of Regulations, (Title 22, Division 12, Chapter 1) are being cited on the attached 9099D.
