Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection on 06/16/22. LPA arrived at the facility at 9:20am. LPA met with Director Jessy Ogbuefi, for the purpose of delivering the findings for the above allegations. Director gave LPA a tour of the facility at 9:30am. LPA observed 10 children with 4 staff members present during the inspection.

During the investigation, interviews were conducted with Director, staff (2), children (2) , and parents (7).
On 03/29/22, during 10-day inspection, LPA observed water rings on the lower part of the wall between the bathroom and classroom. Per interview, Director confirmed a water pipe burst on 3/22/22. Although water pipe burst unexpectedly at the facility, the staff closed affected area and closed the facility for a period of three days for repairs the following day. LPA reviewed receipts from work performed dated 03/23/22 to 03/25/22. Although the pipe was fixed, the facility was not tested for mold nor did any interviews conducted confirm the observation or smell of mold.

Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection on 06/16/22. LPA arrived at the facility at 9:20am. LPA met with Director Jessy Ogbuefi, for the purpose of delivering the findings for the above allegations. Licensee gave LPA a tour of the facility at 9:30am. LPA observed 10 children with 4 staff members present during the inspection.

During the investigation, interviews were conducted with Director, staff, children, and parents. Receipts were reviewed. On 03/29/2022, during the 10-day inspection, LPA observed uneven pavement underneath the outdoor play structure located in the children’s play yard posing a tripping hazard. There was a closed bathroom stall in the children’s restroom due to toilet being inoperable. In the kitchen, LPA observe paint chipping above the refrigerator where food is prepared for children in care. The preponderance of evidence standard has been met, therefore, the above allegation is found to be substantiated. California Code of Regulations, (Title 22, Division 12, Chapter 1) are being cited on the attached 9099D.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by facility tour on 03/29/22. LPA observed uneven pavement in the play yard. There was an inoperable children's bathroom stall and there was paint

During today's visit 06/16/2022, LPA observed all the children's bathroom stalled to be in good repair and LPA also observed the all uneven pavement was removed and replaced by new turf. Pictures were taken by LPA. Per Director will have paint chipping fixed and send LPA pictures by POC due date of 07/07/2022.
