On 11/14/22 at 12:00 PM Licensing Program Analysts (LPAs) Austin Estrada and Susann Sanchez conducted an unannounced Required Annual Inspection at the above facility. LPAs met with Director Jessy Ogbuefi and informed her of the purpose of the visit. Director provided a tour of the facility. Per Director Jessy Ogbuefi, the hours of operation are Monday - Friday 7 AM to 5 PM. The facility has three classrooms in total. Per Director, classroom 1 is not being used at the moment. Care is provided to children in classroom 2 and 3. LPAs observed 4 staff and 9 children present at the time of inspection. Teacher child ratios were observed and staff names recorded. The following was observed during the tour of the facility:

Sign in and out takes place at the main entrance area. LPAs observed 4 of 9 children present were not signed in, posing a potential risk to the safety of children in care. All areas identified on the facility sketch were toured both indoors and outdoors.


LPAs observed age-appropriate furniture, toys, and equipment to be in good condition, free of sharp, loose, or pointed parts accessible to children. LPAs observed individual cubbies with children names to store personal items. LPAs observed cleaning solutions stored inside a locked storage closet in classroom 3 inaccessible to children in care.

LPAs observed 2 bathrooms (in classroom 2 and classroom 3) each with 4 toilets and 4 sinks. LPAs observed the bathrooms to be in good condition with working toilets, sinks, hand soap dispenser, paper towels.

LPAs observed the outdoor play yard and the equipment to be age appropriate and in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, are cushioned with cushion padding that absorbs a fall. LPAs observed the play structure to be age appropriate. For outdoor drinking water, a water container with disposable cups are provided for children. LPAs observed adequate shade in the play yard. Fencing around the play yard was inspected and in good condition.

LPAs observed the kitchen area, food preparation area and food storage area to be kept clean and are free of litter, rubbish, rodents, and/or any other vermin. LPAs inspected the food preparation area and refrigerator to be clean and in operable condition. Food was observed not to be expired. Food that was not in the original packaging were sealed and labeled with an expiration date. Per Director, children are provided with breakfast, lunch, and an afternoon snack. LPAs observed trash bins with tight-fitting covers that are kept on, and in good repair.

Per Director, there are no weapons or bodies of water on the premises. Carbon monoxide detector and smoke detector were tested and are in working condition. A 2A10BC fire extinguisher was located in the kitchen area indicating fully charged and serviced on 11/21. LPAs advised the Director to purchase a new fire extinguisher or get the existing one serviced as they are to be serviced yearly. LPAs reviewed the Earthquake & fire drill log last conducted in September of 2022. First Aid kits are located in each classroom and were inventoried.

Napping equipment and bedding were inspected in each classroom. Per Director, linens are washed weekly by parents. Inspection was conducted for appropriate storage and cleanliness of children's blankets and personal items. For water drinking LPA observed disposable water cups and water pitcher in each classroom. Per Director, There is a isolation area in each classroom away from children. A couch is provided for ill children waiting to be picked up. Per Director, if a child is ill, one toilet and sink will be reserved for the ill child.

LPAs reviewed staff and children files. LPAs observed 3 out of 4 staff members present during the inspection did not have their Mandated Reporter Training Certificate. Children’s files were reviewed and observed to be complete.



Per Director, no children in care require medication. If a child does require medication, it is stored in the Directors office inaccessible to children in care.

Lead Poisoning Facts Flyer (PUB 515) – All licensees (Child Care Centers) must provide the following flyer to parents and families upon enrolling or re-enrolling any child. LPAs also informed the Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPAs advised the Director that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Two Type B violations are being cited due to 4 out of 9 children not being signed in at the time of arrival and 3 out of 4 staff not obtaining the Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to a person in care.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview conducted and report was reviewed with Director Jessy Ogbuefi.

The Director was provided a copy of appeal rights.
