Licensing Program Analyst (LPA) Jennifer Hua conducted an unannounced Required - 1 year inspection. LPA met with director Melissa Garcia. A COVID-19 risk assessment was conducted. LPA provided director with a copy of the LIC 125 Entrance Checklist to facilitate the inspection. This is a preschool program. Business hours are from Monday-Friday, 6:30 AM to 6:30 PM. Per director, the facility provide service to military families.

Director took LPA on a facility tour at 8:54am. All areas identified on the Facility Sketch were inspected. There are of total of 4 classrooms for the preschool program. LPAs observed the following: Pre-K room- empty, PS-B - 17 children with 2 staff, the 2s room - 18 children with 2 staff, PS-A - empty. Director also assist in the classroom as needed.

The facility was observed to be within the license capacity and limitations. Facility roster was reviewed. . Sign-In and Sign-Out sheets were reviewed. Per director, parents sign in/out on tablet with QR code App call MCT Care Connect using their individual PIN #, documents can be printed as needed.

Rooms identified on facility sketch were inspected Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was observed. Snack/lunch menus reviewed and are posted.

A first aid kit is kept in each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers are serviced annually. Drills were conducted on July 7,2023. Storage cabinet in classrooms was locked and hazardous items including cleaning compounds were stored inaccessible to children.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. Children may bring their own water bottle or facility will provide. Play area was inspected for hazards and inaccessibility to bodies of water. No hazards observed.

Teacher--child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Personal Rights of children were observed by LPAs. Staff and children Records were reviewed for completeness. Inspection of required forms made.

Staff and Children’s Records were reviewed. Criminal Record Clearances were reviewed for Associations and Transfers. Director and Staff are current on Pediatric First aid/ CPR as observed. staff records reviewed have required Immunization in file. Staff have completed the Mandated Reporting training.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children's, personnel, and administrative records. The following was discussed with director:

Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Deficiencies cited on attached 809D

Upon receipt of this report documenting a substantiated complaint allegation and a Type A deficiency, the licensee shall do the following:

1. Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency.


2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit.
Exit interview was conducted with director Melissa Garcia, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. Per director water lead test was done in October 2022, will submit copy of lab test result and facility sketch to LPA by 8/3/23.

Based on observation,interview and record review, the licensee did not comply with the section cited above, during the tour with director, LPA observed in the PS-B classroom, 17 children supervised by 1 teacher and 1 aide, in the Twos classroom, 18 children supervised by 1 teacher and 1 aide. PS-B classroom was out of ratio by 2 children, Twos classroom was out of ratio by 3 children. which poses an immediate health, safety or personal rights risk to persons in care.
