On December 6, 2019 at 9:15AM, Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual inspection. LPA met with Diane Peyton, Site Director, who guided analyst on a tour of the facility. This is a preschool program, children ages 2 until age 5. Facility operation hours are Monday to Friday from 6:45 AM to 6:00 PM.

Upon arrival, the following staff were present during the inspection: 4 classrooms: #1 Sunny Smiles, total 16 children enrolled, 9 present today with 2 teachers, #2 Blue Skies, total 16 children enrolled, 9 present today one teacher and one assistant, #3 Children's Place, total 16 children enrolled, 14 present today and two teachers present, #4 Green Trees, total 16 children enrolled, 8 present today with one teacher and one assistant. All areas identified on this report were inspected. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. All children were observed to be under visual supervision of a teacher at all times.

The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair, free of sharp, loose, or pointed parts. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed by the facility weekly. Napping equipment (cots) were observed in separate storage areas. Per Director, the isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms with an igloo dispenser.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that there are no poisons stored at the facility and understands that storage areas for poisons must be locked with a key or combination lock.

The facility has hard wired smoke alarm system and serviced by Bay Alarm Company-South Bay. The system inspector came out to test the equipment on 12/05/2019. There is a functioning carbon monoxide detector that meets statutory requirements.

Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides Breakfast, Lunch, and PM snack. The facility has an on-site cook. LPA observed required licensing documents posted on bulletin board in hallway.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. All foods/beverages capable of spoiling are stored in covered containers at 45˚(F) or less.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment and slides have cushioning material to absorb a fall. The outdoor area had adequate shade. The Director states that there are no bodies of water on the premises and LPA did not observe any bodies of water during this visit. Director states there are no weapons or firearms on the premises.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

The facility was granted a waiver to utilize electronic signatures for the sign in/sign out sheets. LPA observed the electronic sign in/sign out sheets on the system.

Children’s Records were reviewed for completeness; Inspection of required forms was conducted. LPA issued the Children’s Record Review (LIC 857) to the Director during this inspection. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Staff Records were reviewed for completeness; Inspection of required forms was conducted. Children's roster was reviewed and is current. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/16/19.

First Aid supplies were observed in the classroom. According to the Director, medication is only administered to a child when accompanied with a doctor's note and they are stored in the child's classroom in a cabinet where it is inaccessible to children in care. Medication is stored in labeled sealed bags in the kitchen area which is not accessible to children.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Exit interview was conducted with Director, Diane Peyton, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. The Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
