Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required- 1 year inspection. LPA met with Armine Hovakimian, Director at 10:40 a.m. who guided analyst on tour of facility. This is a preschool program which consists of 6 classrooms; RM Group #1 (2-2.5 year old), RM Group #2 (2.5-3 year old), RM Group #3 (Yellow classroom, 3-3.5 year old), RM Group #4 (Red classroom, 3.5-4 year old), RM Group #5 (Green classroom, 4-5 year old), and RM Group #6 ( Blue classroom, 4-5 year old). Facility operation hours are from Monday to Friday from 8:00 AM to 6:00 PM.

All areas identified on the Facility Sketch were inspected at 10:50 a.m.. The following staff was present during this inspection: Group #1: 9 preschoolers with 2 staff, Group #2: 12 preschoolers with 2 staff, Group #3 (Yellow): 15 preschoolers with 2 staff, Group #4 (Red): 11 preschoolers with 2 staff, Group #5 (Green): 9 preschoolers with 2 staff, Group #6 (Blue): 13 preschoolers with 2 staff. The following was observed during tour of facility:

PHYSICAL PLANT: Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed at 11:00 a.m. to be inaccessible to children. Director states at 11:19 a.m. that there is no poison on the premises. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing sinks are safe, sanitary and are operating properly. All floors are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. Kitchen is located between group #1 and group #2 classrooms. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed at 11:20 a.m. to be free of flies, other insects and rodents.

Napping equipment and bedding was inspected at 11:00 a.m. for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Friday and returned on Monday. The Director's office will be used by ill children. There is a napping cot available for an ill child. The staff restroom will used by ill children.

Teacher child ratios were observed at 11:05 a.m.. Sign in and out sheets were reviewed at 11:05 a.m.. During this inspection at 11:21 a.m., there were 69 children present in the facility and all 69 were signed in.

Outdoor playground equipment at 11:05 a.m. is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

First Aid supplies were inventoried. A first aid kit is kept in each classroom. Carbon monoxide detectors and smoke detectors are present and in operable condition. Fire extinguishers have been serviced in 10/16/2019.

Per Director at 11:29 a.m. there are no pets, weapons, firearms or bodies of water on the premises. The facility does not transport children.

FACILITY RECORDS: All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. Educational background, training, mandated reporter training, and/or experience, and health screening form for each staff present are on file and were reviewed. LPA reviewed six teachers files at 11:40 a.m. and there were completed.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. LPA reviewed six random children's files and there were completed.

Staff and Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Confidential Names List (LIC 811) to the licensee during this inspection. The Confidential Names List documents the staff and children’s files that were reviewed during this inspection.

Children's roster was reviewed at 11:44 a.m. and is current. Disaster drill log was available, last drill was conducted on 01/24/2020.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides breakfast, lunch and PM snack.

Medication: Incidental Medical Services (IMS) policy was discussed with director at 11:35 a.m.. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the director at 11:55 a.m. to access forms, child care videos, providers information notices, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: Effects of Lead Exposure flyer was provided to the licensee during this inspection at 11:37 a.m..

There were no deficiencies cited during today’s inspection at 12:00 p.m..



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Armine Hovakimian, Director, at 12:50 p.m. including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
