Licensing Program Analysts LPAs Staicy Perry and Mary Silva conducted an unannounced 3-Yr random inspection to the above facility. At 9:07am, LPAs met with licensee Sonia Flores. Upon arrival LPAs observed 5 children being supervised by licensee, along with assistant #1 and assistant #2 who was cooking in the kitchen. Also present in the home during the inspection was licensees spouse Jose Flores and family member #1 and daughter Leslie Flores. The licensee is primarily Spanish speaking. The licensee states that they currently have 36 children enrolled. A current children’s roster is available and is current. Licensee’s hours of operation are 6:00am-5:45am (23 hour care), Monday-Sunday.

This is a two-story home with four-bedrooms, two playrooms, four bathrooms with one being located outside, kitchen, living room, and 2 dining rooms (formal dining room and informal dining room). This facility has a pool, LPA found that the pool was in compliance per title 22 regulations. LPAs took photos and documented measurements. Per licensee the children do not use the pool. Per Licensee, 4 bedrooms and half bathroom are off limits to children in care. All areas identified on the facility sketch were inspected. The licensee provides food for children in care, breakfast, lunch, PM snack and dinner. The licensee states that 4 adults and 0 minors currently live in the home. Persons living in the home are identified on the attached LIC811. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home. Per licensee, there are no weapons, firearms or bodies of water on the premises besides the pool that has been inspected.



LPAs were led on a tour of the facility by the licensee Sonia. The following areas are used for daycare: Main care is provided in the homes two play rooms (two rooms near the rear of the home), the children use the restroom that is located between the two playrooms and in the backyard when outside. LPAs inspected bathroom for safety, LPAs observed no cleaning compounds were stored in either bathroom. The off-limit areas include the homes, four bedrooms, one bathroom, the formal dining room and the living room.

The home was inspected inside and out for safety, comfort, cleanliness, telephone service (landline), heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds (locked in a closet in the backyard), and hazardous items that can pose a danger to children. Facility rosters complete and maintained current. Fire/earthquake drills current last conducted 11/14/2023. Children nap in the homes two playrooms, LPAs observed children napping in cribs and beds in both playrooms. LPAs observed napping equipment free of extra sheets, blankets and materials that obstructs the vision or airway of the child sleeping. LPAs observed perimeter fencing, and backyard is free of miscellaneous debris/hazards. All trees, shrubs and plants are maintained.

LPAs observed age-appropriate toys and equipment present. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are observed to be posted. All sharp knives are made inaccessible by management safety latch on all drawers in the kitchen. Fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on September 30,2023, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. LPAs discussed the importance of ensuring the fire extinguisher is serviced or a new canister is purchased annually. Bathrooms were free of detergents, cleaning compounds, medications, and other items which could pose a danger are inaccessible to children. LPAs observed the backyard yard area. The outdoor play area was observed to be fenced. Licensee has half of the play yard outside to be shaded and an area which has a play structure with cushion to prevent falls. Licensee does state that her daughter that resides in the back upstairs house who also runs a daycare does share the play yard. Licensee stated that they do not go outside at the same time and that they go off of a staggered schedule. Licensee stated children use the outside area during certain times of the day.

LPA observed perimeter fencing, and backyard is free of miscellaneous debris/hazards. All trees, shrubs and plants are maintained. play yard is used on a staggered schedule M-F from 10:15 am -11:00am and PM from 3:40pm to 4:15pm. Licensee provided declaration confirming this information. LPAs provided licensee with a declaration for her to sign, stating the play yards staggered schedule she shares with her daughter. . LPAs will document declaration in file.

The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. There are first aid supplies available. Smoke and carbon monoxide detectors were tested and are operable. Licensee uses both landlines and cell phone. Per licensee, kitchen lounge chair will be used as an area for an ill child until an authorized representative comes to pick up child. The licensee is observed to be operating within the license capacity limitations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 12/16/24. Licensee has completed mandated reporter training which expires on 8/10/2025. The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation. Assistant Oralia Vasquez and assistant Karina Kruz have completed CPR which expire on 12.16/2024 and Mandated Reporter that expires on 8/10/2025.

-There are 3 small dogs on the premises. That are kept in a cage and outside during the day.


-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family child care facility.
Smoking is prohibited in a license family childcare home.

Licensee Sonia was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.



The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Licensee. The Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.
