Licensing Program Analysts (LPAs) Fabiopla Vasquez and Rita Ramos conducted an unannounced 3 year required inspection. LPAs met with Dandy De Paula, Director who guided analysts on a tour of the facility. The facility operates Monday to Friday from 6:30 A.M to 6:00 P.M.

Upon arrival, the following staff were present during this inspection: Caterpillar Classroom: Staff #1, #2, and #3 with 12 preschoolers. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children have their own cots to nap on. Per Director the sheets are washed weekly.



Per Director the Isolation area for ill children is the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPAs observed a carbon monoxide detector and a smoke detector system. Facility has drinking water accessible indoors and outdoors.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Director states that there are no poisons stored at the facility.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. The cafeteria room has a fireplace that, Per Director, is not utilized whatsoever and it mainly serves as a decoration. LPAs obtained a signed declaration from the Director during the inspection indicating that the fireplace is never used.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. This facility was observed to have two outdoor areas. Currently the children are not using the front yard area. They are using the middle court yard. Per Director, enrollment has been low for several years. LPAs measured the middle courtyard during the inspection and informed the Director that there is only enough space for up to 23 children. LPAs advised the Director that if they are not going to use the front yard they can either not exceed 23 children in care or they can consider reducing their licensed capacity. Per Director, the front yard will be used and the discussion will be made with the Licensee.

All individuals present have obtained a criminal record clearance or criminal record exemption. Staff #1, #2, #3 and #4 are trained in CPR and Pediatric First Aid. The staff records were reviewed during the inspection and the LIC 859 was provided to the Director during this inspection. The LIC 859 documents the staff and their pertaining records. Children’s files were also reviewed during this inspection.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file and they have proof against TB, measles, pertussis, and influenza.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

Children's roster was reviewed and is current, however, LPAs observed that the Director did not have all the required information on the roster. LPAs printed a copy of an LIC 9040 Child Care Facility Roster and advised the Director to include the additional information that is needed. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 06/20/19.

Menus are posted weekly in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. This facility provides breakfast, lunch, and snack.

First Aid supplies were observed in the classroom in a cabinet. According to the Director, currently there is no children on medication, however, medication is administered if a child were to require it and it is accompanied by a doctor's note and parental consent. -------Page 2 of 3

While inspecting the storage of the first aid supplies and the medication, LPAs observed that there is Tylenol and cough syrup that does not have a child's name on it. LPAs consulted with the Director in regards to storing any type of medication whether it is prescribed or over the counter to include a child's name. The medication was observed to be in a storage room inside a high cabinet.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs advised the Director to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov

The following deficiency listed on the attached deficiencies page is being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Dandy De Paula, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
