Licensing Program Analyst (LPA), Bardo Baluyot conducted a Required Annual site inspection. Upon arrival, LPA conducted a COVID risk self assessment and met with Site Supervisor, Maribel Andrade . There were no children present during the inspection. The program operates on the campus of Scott Elementary School. Options Surround Care offers before and after school care. The schedule is as follows: 6:30 AM - 9:45 AM and 1:15 PM - 6:00 PM Monday -Friday (children arrive at 1:35 PM on Tuesdays).

Children attending the before and after school program are housed in separate portables adjacent to the elementary school. Furniture and equipment were inspected for age appropriateness and good repair. The room is well ventilated via central air. The refrigerator, microwave and electric oven were observed to be clean. Carpeting and flooring were also observed to be well maintained. Availability of drinking water is provided via water fountains. First Aid supplies are maintained on site. Cleaning solutions are stored inside a locked cabinets. Children have cubbies to store their personal items in. A variety of games and other learning materials are available for children. Restrooms for children are located inside the classrooms.

Roster was available and up to date. An Emergency Disaster Drill log is kept. Designee is also up to date. The program conducts routine emergency drills (last recorded 9/28). First Aid/CPR observed to be current for both staff. Sign in and out sheets and procedures were reviewed. Children are signed in by parents or staff upon arrival. Children attending the elementary school are singed in by staff. All children physically present were observed to be signed in.

Children use the playground which is adjacent to the elementary school. The playground includes a grassy field and hard yard area where children can engage in sports and social play.

Menus were reviewed for availability, quantity and appropriateness to children in care. The program offers an afternoon snack. Snacks are provided by Options Food Program which follow mandated Federal Food Guidelines. Meal menus, license, car seat law, parents' rights for centers, emergency disaster plan and all other posting requirements were observed.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Child's IMS documentation was reviewed and is complete.

NO DEFICIENCIES ARE BEING CITED IN ACCORDANCE TO TITLE 22 CALIFORNIA CODE OF REGULATIONS.

No children or staff files were reviewed during today's inspection. Files are kept in Options For Learning main office located at: 885 Village Oaks Dr, Covina, CA 91724 (626)858-0527

Exit interview conducted and report was reviewed with the Site Director, Maribel Andrade.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
