A ANNUAL RANDOM INSPECTION WAS CONDUCTED IN ENGLISH

Licensing Program Analysts (LPAs) Ariel Cazares and Thelma Razo conducted an annual inspection on this date. LPAs met with Teacher Lusia Guevara as the director was not present upon arrival. There were a total of 39 children and 4 staff present. Director arrived shortly after. The facility operates Monday-Friday, 6:30am-6:30pm.

The facility was inspected indoors and outdoors. This facility consists of 3 classrooms and one playground. All classrooms were inspected for safety. Furniture, toys, and equipment was observed to be age-appropriate, clean, and in no need for repair. The facility was also evaluated comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. In the nook area near the children's restrooms, LPAs observed 3 drawers with hazards such as lighters, nails, and gardening shears. Napping equipment in form of cots was observed in the classrooms. Linens and bedding is sent home each Friday to be washed. LPAs observed drinking water in form of personal water bottles that can be refilled by the water pitchers. The carbon monoxide detector located in the kitchen was tested and found operational. The children's restrooms were inspected. The toilets and sinks were operational. The hot water is turned off, per director the temperature is too high for children and will need to request from the Church to adjust. The restrooms are stocked with necessary supplies such as soap, paper towels, and toilet paper. Restrooms were observed to be clean.

The outdoor playground was inspected. There was age-appropriate toys and play equipment for children. LPAs observed an awning that provides a shaded rest space. There is a functioning water fountain that can be accessed by children outdoors. Fencing was evaluated and found to be appropriate and no need for repair.

The director's office is used as the ill isolation area and a staff restroom is available. Medication can be stored in the kitchen on top of the fridge. Currently there are no children in care with medication needs. Staff and children's files, sign in and out sheets and rosters were reviewed. LPAs observed that of 39 children present, only 37 were signed in. Child #1 did not have a physician's report/medical assessment on file.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs reviewed postings on the parent board. The facility schedule shows only 1 snack being offered by the facility. LPA informed director that a full LPAs reviewed postings on the parent board. The facility schedule shows day program must provide 2 snacks. Snack menu needs to be updated as posted menu was from 2008.

See attached deficiencies page for citations in accordance with Title 22 California Code of Regulations.

Exit interview conducted with Director Angela Bibile. A copy of this report and appeal rights were provided and explained.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
