An informal office meeting was conducted on 08/28/23 at 11:30 AM at the Monterey Park Regional Office. Present during the meeting were Licensing Program Manager (LPM) Christina Gabelman, Licensing Program Analyst (LPA) Anomeh Eivazian and Licensee Niranjala Bible. The purpose of this Informal Meeting is to discuss the history of the facility.

The summary of history includes 8 Type A's deficiencies from 2022-2015.

Deficiencies include:
On 11/18/2022-- Type A—101229(a)(1)— Responsibility for Providing Care and Supervision. Based on LPA's observation on 11/18/22 at 9:15 a.m. and 9:20 a.m. two children walked in the classroom alone, went to the bathroom alone and then joined teacher Asmik's class.
On 05/16/2022—Type A—101223 (a) (1) -- Personal Rights—On 03/01/2022 LPA Eivazian heard the first hour an half of recording that was provided from complainant from 11/10/2021. LPA heard staff#1 and staff#3 were talking in a proper way with other children, however LPA heard staff#1 and staff#3 talked inappropriate to child#1.
On 06/06/2019—Type A—101223(a)(3) - Personal Rights- Based on LPA's observations of camera footage revealing Staff #1 violating personal rights of 2 children in care.
On 06/06/2019—Type A--101229(a) - Responsibility for Providing Care and Supervision—Child#3 was injured while under staff#2 care and supervision and staff#2 did not observe how child#3 was injured.
On 05/01/2019—Type A—Personal Rights--101223(a)(2) -- LPA observed one child in a crib that has a fan, small play table, a play car structure, and a kid's toolbox in it. This crib was not clutter free and child was in between items. Child was not placed in a safe environment.
On 10/25/2018—Type A--101216.3(a) - Teacher – Child Ratio--LPAs observations of 50 children to 4 teachers.

On 05/24/2016—Type A--101238(g) - Buildings and Grounds--LPA's observed lysol spray, Petroleum painters cover spray were accessible to children.
On 04/23/2015--101216.3(a) - Teacher – Child Ratio--At the time of the visit when teacher # 1 was assisting LPAs with files and tour of the facility acting in behalf of the Director who was not present, ratio in the 3 and 1/2 to 6 years old classroom did not meet the
Teacher # 2 was left alone to supervise (19) children. More children were arriving to the same classroom.


LPM Gabelman informed Licensee that facility has already attended an Informal Meeting on 04/23/2015 and was placed on two years required inspections.

The licensee has agreed to the following to ensure compliance:


1. The facility must be in compliance at all times.
2. The facility will be placed on required visits for one year and an LPA conduct inspection quarterly.
3. The Licensee was advised that it is the responsibility of the Licensee, to know & understand the requirements of Title 22 Regulations.
4. The Licensee will also be required to view the following videos on the Department website https://ccld.childcarevideos.org/:
· Teacher-to Child Ratios in Child Care Centers
· Supervising Children in Child Care Centers
· Children’s Personal Right in Child Care

The Licensee shall submit a written statement to the Department on what she learned from each video no later than 09/29/2023. Licensee will also show the first 3 videos to staff during her monthly staff meeting and submit copy of sign in sheet to the department.
5. The Licensee will complete Child Care Centers Step II Orientation and a copy of certificate will be submitted no later than 09/29/2023.
Licensee was advised if facility is not being met with Title 22 Regulation during the compliance period, Licensee may be called in for Non-Compliance Meeting sooner.
