25 | Licensing Program Analyst (LPA) Seung Lee conducted an unannounced inspection case management inspection. Upon arrival at 12:50PM, LPA was greeted by assistant director Annaliese Miller to whom the purpose of the inspection was explained. At 1:00PM LPA was given a tour of the facility where there were children present at the time of inspection.
Census was taken. All children were napping per schedule. There were 17 children and 1 staff for Lady Bugs (2s), 17 children and 1 staff for Pandas (2s), 12 children and 1 staff for Kangaroos (3-4s), 12 children with 1 staff for Teddy Bears (3-4s), 10 children with 1 staff for Busy Bees (Pre-K), 12 children with 1 staff for Dragons (Pre-K), and 12 children with 1 staff for Zebras (TK).
During an inspection on 07/21/2022, it was disclosed from facility staff, that an incident took place that was not reported to the Department within the required time. The incident took place on 07/07/2022. Facility is required to notify the Department of unusual incidents within 24 hours of becoming aware of the incident. Facility was instructed to submit the unusual incident report. LPA obtained a copy of the written report of the incident that facility had on file.
The following deficiency is being cited in accordance to Title 22 Regulations and or Health and Safety Code (see attached 809D).
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
