Prior to entrance to the facility, LPA Mayra Rivera conducted a Covid 19 assessment and based on the staff responses to the facility assessment questions, LPA Rivera determined safe to proceed. On Monday, July 19, 2021 at 9:45 AM, Licensing Program Analyst (LPA) Mayra Rivera conducted an annual inspection and met with Assistant Director Maria Luengas who guided LPA Rivera on a tour of the facility.

At 12:15 PM LPA Rivera entered the Preschool area to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed 29 preschoolers napping and 3 awake laying in their cot. The following staff were present during this visit: Staff #1, Staff #2, Staff #3 and Staff # 4. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children inside the cabinets with child proof locks. LPA observed age appropriate furniture and equipment in good condition, free of sharp, loose, or pointed parts. At 12:27 PM, LPA Rivera entered the restroom and observed 4 toilets and 4 sinks, hand soap and paper towels. LPA observed the restroom to be in good condition.

At 12:29 PM LPA observed a safety gate in place between the classrooms and the restrooms to prevent children going into the other classrooms. Facility does not provide lunch and LPA observed children's lunch bags stored in their assigned cubbie and inside the lunch bags an ice pack provided by the parents to keep the food cool.

LPA observed the food preparation area and the food storage area clean and free of litter, rubbish and rodents and/or any other vermin. LPA observed trash cans that are used to discard food and waste with tight fitting lids to be in good repair. LPA also observed individually water bottles with children names and water bottles for refills Drinking water is readily available both indoors and outdoors.

LPA observed the tag for the fire alarms and smoke/carbon detectors serviced by the Western States fire protection company on 2/15/21. LPA observed the required 2A10BC fire extinguisher located in the classrooms and the valve on the green area indicating fully charged. LPA observed the earthquake and fire drill log posted on the bulletin board.

At 12:35 PM, LPA Rivera inspected the outdoor area for safety, comfort and cleanliness. LPA observed the outdoor playground equipment in safe condition, free of sharp, lose or pointed parts. The two play structures equipment is by Miracle Recreation Equipment Company for ages 2-5 years. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. LPA observed a sail shade triangle in place and provides adequate shade in the yard. Children bring out their water bottles and refills are provided. LPA advised to place a barrier around the small water fountain to prevent an injury and also to place the cleaning solution a bit higher up. The facility was observed to be free of flies, other insects and rodents.

LPA observed the required 2A10BC fire extinguisher located in the discovery pre-k classroom and the valve on the green area indicating fully charged. LPA observed first aid kids inside the classroom cabinets and the earthquake and fire drill log posted on the bulletin board.

At 1:00 PM LPA reviewed facility records. All individuals present have obtained a criminal record clearance or criminal record exemption. Staff #1 and Staff #2 present during this inspection currently have a valid pediatric first aid/CPR certification. LPA observed proof of immunization against pertussis, measles and influenza and influenza declination. LPA also observed educational background, training, and/or experience for each staff present.

LPA reviewed children’s records. LPA observed the required documentation such as the LIC 700 Emergency Information , LIC 701 Physician's Report, LIC 702 Child's Preadmission Health History, LIC 627 Consent for Emergency, LIC 613A Personal Rights, LIC 995 Parent's Rights, Immunizations, and LIC 9227 Individual Infant Sleeping Plan/Services Plan The files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

LPA observed the snack menus posted on the bulletin board and are posted monthly in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. The facility provides AM snack and PM snacks.

LPA guided director Edith Martinez and Assistant Director Maria Luna in how to use Guardian. LPA explained the purpose of utilizing guardian and the procedure in how to associate staff to the facility.

Incidental Medical Services (IMS). Director states that prescription medications are administered and offer incidental medical. LPA observed the medication stored in directors office. LPA observed child # 5 medication Ventolin with expiration date 8/18/21, child # 6 medication pain reliever with expiration date 11/2/21 and child # 7 asthma inhaler with expiration date 8/28/20. LPA informed director child #7 medication has expired and to inform parent to bring a new inhaler.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA Rivera issued the Confidential Names List (LIC 811) to Director during this inspection. The Confidential names list, documents the children’s and staff files that were reviewed during this inspection.

At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

LPA provided The Notice of Site Visit (LIC 9213) and explained that it must remain posted for 30 daysduring the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director Edith Martinez. LPA explained and discussed this report, including the appeal rights and provided a copy.The signature on this report confirms receipt of these documents.
