On January 30, 2025, at 9:00am, Licensing Program Analyst (LPA) A. Carter conducted an unannounced Annual Random inspection and met with Director, Alexis Landa. LPA disclosed the purpose of the inspection and was granted entry unto the facility.

LPA was taken on a guided tour of both the indoors and outdoors of the facility. Teacher-child ratios were observed, and staff names recorded. All center staff present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. The sign-in and out sheets were reviewed. Personal Rights of children were observed by LPA.

The hours of operation are 7:30am – 2:45pm, Monday - Thursday, Friday 7:30am -12:00pm. Facility is currently licensed for a capacity of 20. At the initial start of the site visit, there is a total census of 7 children and 1 teacher present. This is a preschool program made up of 1 classroom. LPA observed all posting requirements for operation on the posting board and the following documents were observed: LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in classroom. The isolation area used is in the office for sick children. Children napping equipment met the requirement regulation at time of inspection. Children bring their own bedding, and it is sent home on Thursday to be cleaned and returned on Monday. Children have individual storage for their belongings. The facility has a waiver on file which grants permission to use the same restroom as the elementary school on the premises. Staff accompany children during use of the restroom.

Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. LPA observed 8 toilets and 8 sinks. LPA observed first aid container kept in classroom. First Aid supplies were inventoried and were found to be in compliance with regulation.
The facility provides two snacks and children bring lunch from home. Food and snack items are properly stored. Availability of indoor drinking water was observed in classrooms via water bottles and a portable water dispenser with a separate water container. Per Director, Staff assist children with the drinking water if they need to re-fill their water bottles. Per Director, the preschool does not use any water source for drinking water or food preparation. LPA confirmed Carbon monoxide and smoke detectors were present and determined to be in operable condition. Fire extinguisher was observed as serviced on August 1, 2024. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. This facility does not have any children with allergies or needs medication administered. LPA observed Disaster Drill log with last drill on October 18, 2024
At 9:45am, outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. There is also a waiver for the outdoor play area. Outdoor play area is shared with the elementary school on site. Children are supervised while using the outdoor play area. LPA observed sensory tables, a play structure with a plastic slide, and other age-appropriate toys around the play yard. Required shade and fencing were inspected. The children have access to drinking water via water bottles observed by LPA. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.
At 10:00am, staff and children’s records were reviewed. Mandatory forms for the children and staff files were discussed. LPA observed current CPR and First Aid Certification for center staff. CPR card expires on 02/05/26 for Staff#1 (S1). Role and responsibilities of being a Mandated Reporter were reviewed. S1’s Mandated reporter training expired on 03/21/24. A new certification is required.
Children’s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report, and Immunizations Records. All required documents on file.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
