An unannounced Annual inspection was conducted on this day by Licensing Program Analyst (LPA) Jacqueline Martinez. Facility is currently licensed for a capacity of 75 preschool children. LPA met with Maria Luisa Moreno, Site Director who guided analyst on a tour of the facility. This is a Child 360 preschool program which is operated by YMCA of Greater Long Beach and is located on the premises of One Church. The program consists of a full day program in Room 1 and two part-time day programs in Room 2 & 3. Days & Hours of Operation are M-F; full day program from operates from 7:00am to 5:00 pm in Room 1. The AM Session operates from 8:00am to 11:15am and the PM Session operates from 12:45pm -4:00pm in Room 2 and 3.

All areas identified on the Indoors and Outdoors Facility Sketch were inspected. LPA observed a total of 30 children, and 25 who were napping and 5 children who were awake with 1 Staff member, Staff #2. Site Director was in the room not directly supervising the children but behind her computer working. Within 6 minutes Staff #3 arrived and within 8 minutes Staff #4 arrived. In Room 2 LPA observed 20 children with 1 Teacher, Staff #5 and 1 Teacher Assistant Staff #6. Facility failed to follow staff and child ratios posing in Room 1 and 2 during this inspection, posing an immediate safety risk to the children in care. A Type A citation is being cited on the LIC 809-D page. In Room 3 there 12 children with 1 Teacher. Teacher child ratios were observed and staff names recorded. Sign in and out sheets and procedures were reviewed.

Furniture and equipment was inspected for age appropriateness, good repair and free of sharp, loose, or pointed parts. Telephone service, heating, lighting and ventilation were evaluated. Napping equipment (cots) and bedding were inspected for good condition, appropriate storage and cleanliness. Children take their blankets and sheets home to wash weekly. Storage for children's belongings and isolation area were inspected. Ill isolation area is in the Main Office. Drinking water is readily available indoors for children to drink freely via a water jug and disposable cups.

Incidental Medical Services (IMS). Incidental Medical Services (IMS) policy was discussed. The facility is providing IMS Services however there is no IMS Plan on file. This was discussed with Site Director, Maria Luisa Moreno. A review of the medication policy, including administering, labeling, storage, and records was made. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided; US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

During the review of the medication, LPA observed that Child #1 who was terminated in June 26, 2018, still had their medication at the facility. Child #4 who was terminated on July 20, 2018 still had their medication at the facility. Facility failed to return Child #1 medication to the children parents, once the children were no longer attending the program. Facility failed to dispose of the medication or return the medication to the children's authorize representative. A Type B deficiency is being cited on the LIC 809-D page.

AB1207: This is a reminder that, As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

LPA observed that 7 out of 8 Staff members present did not have proof of the AB1207 Mandated Reporter training certificate during this inspection. A Type B deficiency is being cited on the LIC 809-D page.

LPA advised that no children shall be left without the supervision, including visual supervision, of a teacher at any time. LPA advised to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

An exit interview was conducted with Maria Luisa Moreno, Site DirectorA copy of this report and Notice of Site Visit was provided. Appeal rights were issued and discussed.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Upon receipt of this report, the Licensee shall do the following:
1.Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency in each room and in a place where its visible to the parents who come into each room
.
2.The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.

3.A copy of this report along with the LIC 809-D pages shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).

4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form and Appeals Right was provided during this visit.

5. Post the POC once it is cleared for 30 days.

LPA observed that none of the rooms inspected had a carbon monoxide detector. LPA inquired about the carbon monoxide detectors with Site Director, who stated that there are no carbon monoxide detectors at the center. This poses a potential safety risk to the children in care. A Type B deficiency is being cited on the LIC 809-D page.

Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. First Aid supplies were reviewed and inventoried.

Food preparation and storage areas were observed to be clean, free of litter, rubbish and free of rodents and other vermin. Food is provided by Shamrock and US Foods. The facility provides, breakfast, lunch and a PM snack. Proper storage of food, beverages and snacks was reviewed. Food menus were posted in a place that is visible by the child’s authorized representative. Storage containers for solid waste, including moveable bins had tight-fitting covers that are kept on, and in good repair. All floors were observed to be clean and safe. Disinfectants, cleaning solutions, medications and other items that are dangerous to children, were inaccessible.

Outdoor play equipment was inspected for safety, free of sharp, loose pointed parts, good repair and age appropriateness. The surface of the outdoor activity area was observed to be maintained in a safe condition, and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. Required shade, drinking water and fencing were inspected. Drinking water was available outdoors for children to drink freely via a water fountain. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards were observed.

Children's records were reviewed as well as a sampling of children’s emergency information. Staff files were also reviewed.. If there are any deficiencies during that review, this report may be amended. Staff #1 and Staff #2 provided proof of current Pediatric First Aid and CPR certification. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence has been reported to the department.
