An unannounced Annual Random Inspection was conducted on this day by Licensing Program Analysts (LPAs) Carolyn Tuba and Nolan Tcheng. Facility is currently licensed for a capacity of 66 children. LPAs met with Maricruz Flores, Director, who guided analysts on a tour of the facility. Facility is fenced and gated.

The Preschool program consists of a full day session from 6:15 am to 6:00 pm and a half day session from 8:00 am to 12 pm. The facility operates from 6:15 a.m. to 6:00 p.m. from Monday to Friday. The indoors and outdoors of the facility were inspected. LPAs observed 23 children 2 Staff Members in Room: 4's (Pre-K); 10 children with 1 Staff Member in Room:3's and 16 children with 2 Staff Members Room:2's. Teacher child ratios were observed and staff names recorded. LPAs reviewed sign in and out sheets and found 47/49 children were signed in. After discussing with Director, 2 missing signatures were written on the wrong date.

Furniture and equipment were inspected for age appropriateness, good repair and free of sharp, loose, or pointed parts. Telephone service, heating, lighting and ventilation were evaluated. Napping equipment (cots) and bedding were inspected for good condition, appropriate storage and cleanliness. Per Director, the napping sheets and blankets are laundered daily at the facility. Storage for children's belongings and isolation area were inspected. The ill isolation area is in the Director’s Office. Drinking water was observed to be available indoors for children via a water jugs and disposal cups. Water fountains are not in use right now due to Covid.

Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. First Aid supplies were reviewed and inventoried.

Food preparation and storage areas were observed to be clean, free of litter, rubbish and free of rodents and other vermin. Children bring their own lunches. All storage containers for solid waste, including moveable bins had tight-fitting covers. All floors were observed to be clean and safe. Disinfectants, cleaning solutions, medications and other items that are dangerous to children, were inaccessible.

The outdoor play area were inspected and is fully fenced. Required shade, drinking water and fencing were inspected. Drinking water is available outdoors for children via a water jug and disposable cups. The outdoor play area was inspected for bodies of water; no bodies of water were observed.



Children’s files were reviewed for proper documentation and found to be complete. Staff files were reviewed. Staff #6 the Health Screening (LIC 503) form was not on file. Staff #1, Staff #3 Staff #7 had expired AB 1207 mandated reporting training. All staff present are fingerprint cleared. LPAs verified the opening and closing staff have proof of current Pediatric First Aid and CPR certification. The child care center director and teacher(s) have been verified to be fully qualified and assistant director is qualified to act in director's behalf.

Incidental Medical Services (IMS). The facility does not provide Incidental Medical Services-IMS. AB1207: The following was discussed as a reminder: As of January 1, 2018, Health & Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be met by using the
Department’s Office of Child Abuse Prevention (OCAP) online training modules at: http://www.mandatedreporterca.com/.

Based on this information, the following deficiencies listed on the attached LIC 809D and LIC9102 are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

At 4PM an exit interview was conducted with Maricruz Flores, Director. A copy of this report and Notice of Site Visit was provided.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
