25 | Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 8/25/23. A COVID risk assessment was conducted. LPA met with Director, Maricruz Flores to whom the reason for the visit was explained. Census was taken. There were 7 staff present caring for 44 children. The facility was observed to be operating within the license capacity limitations.
On August 31, 2023, an unusual incident report was made to the Department regarding an incident that involved a child who stated a teacher pinched them on the cheek. Based on information obtained during interviews conducted with staff, LPA Babcock determined there was not a preponderance of evidence to prove that the teacher did in fact pinch the child. The child was not at the facility on the date of LPA visit, and therefore could not be interviewed. Based on interviews with staff, there was adequate supervision at the time of the incident.
Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported.
No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with Director, Maricruz Flores.
