On 8/29/23 at 1:15 pm, Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced annual inspection. A COVID risk assessment was conducted. LPA met with Education Coordinator, Richard Mata to whom the reason for the visit was explained. Mr. Mata guided LPA on a tour of the
facility. This is a before and after school program operating on the campus of La Colima Elementary School. Hours of operation are Monday through Friday, 6:30am - 8:00 am and 1:15pm - 6:00 pm. Facility is currently licensed for a capacity of 40 children. Per Education Coordinator there is one child
enrolled.

Census was taken upon arrival and LPA observed one child being cared for by Mr. Mata. No other staff or children were observed by LPA. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. LPA confirmed all individuals present have obtained a criminal record clearance. The child present was observed by LPA to be under supervision, including visual supervision, by Mr. Mata at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

All areas identified on the Facility Sketch were inspected and the following was observed: The facility operates in one room for the school-age program. Furniture and equipment are age-appropriate and in good repair, and the floors have a surface that is safe and clean. Storage for children's belongings were inspected and LPA observed children have cubbies to store their belongings. Children bring water bottles from home and are allowed to drink freely at any time. demand from the Britta container located in the classroom. Per Mr. Mata, the facility opened on 8/28/23 after being closed since 2020 due to COVID and lack of enrollment and lack of staff. Mr. Mata stated a teacher filled the Britta container for him and he is unsure where it was filled from. Per Mr. Mata, the facility has not been tested for lead and LPA did not find any testing results on the lead portal.

LPA observed toys and materials for children in the classroom. Age appropriate sinks and toilets were inspected for availability and good repair in restrooms used by children. Per Mr. Mata, one area of the classroom is used as an isolation area and staff provide 100% supervision when ill child is using the isolation area and/or restroom, and parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Food menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Mr. Mata, the facility provides PM snack only and uses a "Food Program".

All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Mr. Mata states that disinfectants and cleaning compounds are stored in locked cabinets. LPA observed cabinets to be locked. Mr. Mata states that there are no poisons or firearms on the premises.

Per Mr. Mata, there are no children that currently use medication. The first aid kit in the classroom was observed to have complete supplies.

Telephone service, heating, lighting and ventilation were evaluated. Per Mr. Mata the facility has their own land line.

LPA observed the facility to have operable carbon monoxide and smoke detectors on this day.

The facility has an outdoor play area that is fully fenced. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are safe toys, play equipment and materials observed for children outside on playground. Availability of outdoor drinking water was observed.

Sign-in and Sign-out is done digitally through the HUBBE program. Parents and authorized representatives sign their name on the HUBBE program. LPA observed all required posted forms to be posted by the facility door.

Staff Records and Children’s Records were reviewed. As only one child is enrolled, LPA reviewed the child’s file and observed it to be complete. LPA reviewed the staff file for Education Coordinator, Richard Mata and it was complete. LPA observed Mr. Mata is trained in Pediatric First Aid and CPR, and has current Mandated Reporter Training (AB1207).

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Based on LPA’s observations and record reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22.
Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
