At 9:13a.m. on 6/25/2021, Licensing Program Analyst (LPA) Lissete Gonzalez conducted an unannounced Case Management Licensee Initiated inspection. LPA met with Child Development Program Director, Rosa Ducoing. There were zero (0) children present. LPA conducted the inspection for the purpose of addressing a Toddler Option addition. Applicant is requesting 12 Toddlers and 20 Preschoolers for a total capacity of 32. The facility is currently licensed for 36 Preschoolers.

The proposed Toddler classroom is Room #B. At 9:36a.m., LPA began facility tour with the Child Development Program Director. All areas identified on the facility sketch were inspected. Room #A is a preschool classroom and is adjacent to Room #B. The children's restroom is located in between Room #A and Room #B. LPA did not observe a child safety gate in Room #B to prevent the toddlers from accessing the preschool Room #A.

LPA observed age appropriate equipment, toys, and supplies for children. Room #B (Toddlers) has designated an isolation area in the classroom with a mat available for ill children waiting to be picked up. There are napping cots available for the children. Per Child Development Program Director, linens and bedding will be sent home for laundering every Friday. There are laundry services available at another facility location to be used as needed. There is one changing table located in Room #B within arm's reach of a sink.

The restroom was observed to be clean and hazard free. LPA observed a landline in Room #B. LPA observed the smoke and carbon monoxide combination detector in the Room #B to be operational. There are emergency supplies and a first aid kit located in the toddler classroom. There is a parent board with the required postings located outside the classroom. The children will be signed in and out manually.

There is drinking water available in form via a water jug with disposable cups. Cleaning supplies are kept in upper cabinets that are locked in the kitchen area. There are trash cans with tight fitting lids available in the

classrooms. Central air and heating are available in the classrooms.

LPA did not observe a menu posted in the classrooms due to current facility closure for Summer break. Facility staff advised menus must be posted monthly when school is in session. Meals will be prepared and delivered daily by the Bassett Unified School District cafeteria. There is a food preparation area located in between Room #A and Room #B. There is one refrigerator designated for Preschool and Toddler children and storage space for supplies located in the food preparation area. The kitchen was observed to be clean and orderly.

The proposed Toddler outdoor playground was inspected and measured. There is age appropriate play equipment in the playground. There is a fence separating the Preschool playground from the Toddler playground. The outdoor space is fenced. Drinking water is available in form of water fountain. There is adequate shade in the yard.

Based on the measurements, the facility can accommodate 20 Preschoolers and 12 Toddlers. There is a current and valid fire clearance on file with the Department with an inspection date of 5/19/2021.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual -
Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is
provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding

LPA observed the preschoolers and toddlers will be sharing the restroom. LPA observed the toddlers will need to walk through the preschool playground daily for drop off and pick up. LPA observed the preschoolers may need to walk through the toddler outdoor playground located at the exterior of Room #A in the event of an emergency. The matter will be addressed with the management team prior to approval of granting the toddler

No deficiencies cited during this visit.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
