An unannounced Required One Year Inspection was conducted on this day by Licensing Program Analyst (LPA) Raul Navarro. Facility is currently licensed for a capacity of 49 School Age children. Licensing arrived at about 02:05 pm and met with Director Lyanne Anniuu. Children arrived at about 3:00pm. The program currently operates Monday thru Friday from 2:30am to 6:00pm. LPA observed nine children in care with two staff at 3:00pm when children were present.

A tour of the facility was taken at about 2:30pm. LPA toured the School Age classroom and did not observe any children or staff present as children had yet to arrive from school. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Availability of drinking water was reviewed. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Materials for the children were observed to be clean.

A first aid kit is kept in the classroom. Carbon monoxide detectors and smoke detectors are present in the facility . Fire extinguishers have been serviced. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness at about 2:35pm. Required shade, drinking water and fencing were inspected. LPA Navarro advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times.

The children have access to drinking fountains during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment. The facility provides an afternoon snack that is brought from an outside vendor.

Sign-in and out procedures were reviewed. Staff and Children’s Records were reviewed at about 3:30pm. Criminal Record Clearances were reviewed for adults. LPA observed that one staff was not associated to the facility. Per Director, staff has been fingerprint cleared through the city of Carson, however, they are not associated to the facility. CPR/First Aid card provided for Director and her staff present were expired. Inspection of other required forms was made.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The deficiencies listed on the following page were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public- information-and- resources/safe-sleep.as an additional resource. LPA also informed licensee/facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website athttps://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Lyanne Ainuu.
