25 | Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management inspection. This inspection is regarding a personal rights incident that took place on September 6, 2023. LPA met with Associated Director Monica Gonzalez who was assisting the facility's operations at this time.
LPA observed the following infant classrooms: Infant Room 1 (One teacher, four infants), Infant Room 2 (One teacher, four infants), Infant Room 3 (Three teachers, 10 infants), Infant Room 8 (Two teachers, eight infants) and Infant Room 9 )Two teachers, seven infants). The facility ratios are in compliance
LPA also reviewed training protocol for infant pick up and was informed of the following protocol:
1. Check visitor Identification and make copies 2. Assign visitors an in and out code.
3 Escort to classroom. 4. Ask what child they are picking up.
5. Communicate to the teacher. 6. Teacher does name to face
7. Admin does name to face check 8. All Management available does name to face check before release.
The following item is cleared Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. This report will be sent to the Licensee via email with a read receipt to confirm receipt of the report. Exit interview was conducted with Director Monica Gonzalez. |
