Licensing Program Analyst (LPA) Warren Birks conducted an Annual Required inspection. LPA met with Assistant Director Vivian Franco who guided analyst on tour of the facility. This is an infant program serving infants (zero to two years old). The program is physically separate from a preschool and toddler program housed on the premises. The facility operates Monday through Friday from 6:30 AM to 6:30 PM.

At approximately 11:30am all areas identified on the Facility Sketch were inspected. LPA observed the following teacher-child ratios during the inspection: Infant Room 1: (3 teachers and six infants), Infant Room 2 (One teacher and four infants), Infant Room 3 (2 Teachers and six infants), Infant Room 9 (3 teachers and ten infants), Infant Room 8 (One Teacher and 4 infants). Teacher-child ratios were in compliance and staff have the required clearances at this time. LPA informed Assistant Director to ensure ratios are constantly maintained at a one to four ratio.

LPA observed and inspected all furniture and equipment for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were observed to be satisfactory. Storage for infants’ belongings and cribs were inspected and found to meet all regulatory requirements at this time. Classrooms use a changing table with 3 inch sides and a 1 inch changing pad. LPA also observed the changing are within arm’s reach of a hand washing sink. LPA observed toys to be clean and appear age appropriate. LPA informed Assistant Director to ensure that LPA observed first aid kits kept in each classroom. Carbon monoxide detectors and smoke detectors are present in the facility. 2A10BC Fire extinguishers are due for service in February 2024.

Hazardous items such as medicines and cleaning compounds are stored in high cabinets and/or storage rooms inaccessible to children. The office is used as an isolation area and portable mats for resting and changing are available if necessary. The facility provides formula, snacks, and lunch for the children. Parents also provide food and breast milk. LPA observed the kitchen and food storage area appear to be clean and sanitary food with an adequate amount of food on on hand.

LPA observed drinking water in each classroom and children have their names labeled on their bottles and cups and food from home. Formula and breast milk have labels (along with child information listed) to distinguish one from the other. LPA observed cheese and deli meat placed in plastic bags for snacks (expiration label blank). Assistant Director Franco indicated that the food was being served for snack today. LPA issued a technical advisory violation due to no date labeled on the new packaging.

At approximately 12:00pm the outdoor play equipment was inspected for safety, cushioning material, age appropriateness and good repair. LPA observed the outdoor play area to be physically separate from the preschool play area. LPA observed the infant playground to have adequate shade and adequate fencing. There were no bodies of waters and no appearance of immediate hazards on the playground. LPA Birks reminded Assistant Director that infants need to have direct care and supervision, including visual supervision of staff at all times. Note: LPA informed Assistant Director to ensure that leaves and debris are monitored especially after rain.

LPA evaluated care and supervision and determined the basic needs of infants were met. LPA observed digital Sign in and Sign out records. At approximately 1:30pm LPA observed six of six files to be in compliance during a Children's Records Review. The review ensures children files have proper forms (such as Identification and Emergency, Immunization records, Health History, Personal Rights, Parents Rights, Needs and service plans, Infant safe sleep forms (facility logs 15 minute checks digitally).

At approximately 1:00pm LPA reviewed Staff Records to ensure: All individuals present have criminal record clearance (or criminal record exemption), Staff 8 missing infant and ECE units (education), Criminal Record Clearance documents, current CPR and First, proof of immunization against pertussis and measles and influenza (or flu declination available) etc. LPA observed at least one staff member to have current CPR (Expiration in 2025).

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following was discussed with Assistant Director: Recently Approved Safe Sleep Regulations in Effect (PIN 20-24-CCP). LPA also informed Director to have staff sleep infants on their backs and check on them at a minimum at least every 15 minutes to observing signs on distress, labored breathing, flushed skin color, increase in body temperature and restlessness).

Rooms that are off-limits need to always be inaccessible during operating hours. Smoking is prohibited. No infant walkers, no infant bouncers, no Johnny Jumpers, no excer-saucers or any other item that falls into that category are allowed in facility. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Last drill conducted November 2023. Role and responsibilities of being a Mandated Reporter were reviewed. The Assistant Director was advised how to access forms and Regulations online at www.ccld.ca.gov. Site Supervisor was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.


LPA advised Assistant Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check. Records for all children and staff must be maintained for three (3) years after separation from the facility and report Unusual Incidents to parents/guardians and Licensing within the time frame specified by the regulation. Exit interview was conducted with Assistant Director Vivian Franco The facility was provided a copy of their appeal rights (LIC 9058). The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
