On 06/08/2022 at 11:30 am, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced inspection at the facility noted above and met with Director, Sonna Elliott. The purpose of the inspection was to conduct the Required - 1 Year inspection. The preschool is located at Lakewood First Methodist church. At the time of the inspection, 7 staff and 24 children were present. The hours of operation are Monday through Friday from 7:00 am to 6:00 pm.

All areas identified on the Facility Sketch were inspected. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings and an isolation area with sink, toilet, and cot was inspected. First Aid supplies were inventoried. Every classroom has a first aid kit. Availability of drinking water was reviewed. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, paper towels, area safety and sanitation.

Per Director, the facility does not provide meals or snacks upon reopening due to COVID-19. Children are required to bring their own meal and 2 snacks from home each day. The facility kitchen was toured for safety, cleanliness and proper equipment. Per Director, no poisons, firearms or weapons are kept on the premises. Medication is securely stored in the office. Medication that requires refrigeration is kept in the refrigerator located in the staff lounge or kitchen. At this time, no children require medication.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. The following documents are posted in a prominent, publicly accessible area at the facility: Facility License, Personal Rights (LIC 613A), Child Passenger Restraint System Poster (PUB 269), Notification of Parents' Rights (LIC 393), and Emergency Disaster Plan (LIC 610).

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Staff was questioned to establish their familiarity of emergency reporting requirements, emergency disaster plans and other site operations. The disaster drill log was readily available for review. Sign in and out sheets were reviewed. Staff procedures for checking children for illnesses was discussed. The facility has hand sanitizer, gloves, and face masks readily available. Transportation is not provided by the facility.

Staff records and Children Records were reviewed. Criminal Record Clearances for adults, and verification of Pediatric CPR/First Aid (EMSA-approved) was reviewed. At least one staff has a current Pediatric CPR/First Aid at the time of the inspection.

During the record review, LPA found that 5 staff did not have record of influenza vaccination, 2 staff did not have record of immunization against pertussis, 1 staff did not have record of immunization against measles, 6 staff do not have record of completion for mandated reporter training (AB1207), and 1 staff did not have on record a Health Screening Report (LIC 503). Deficiencies were cited (refer to LIC 809D).

Director was informed that the mandated reporter training (AB 1207) must be completed every 2 years using the following weblink: www.mandatedreporterca.com. Director was also informed that staff may access EMSA-approved Training Programs for Pediatric First Aid & CPR and Preventative Health & Safety through the following weblink: https://emsa.ca.gov/childcare_provider1/.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days.
