32 | Child#1 had a hard time for transition from outside to inside. Per staff interviews, child#1 was hitting with hands on forehead when would not get what asked. Also, child#1 hit teachers and child#1 would put their body on the outside ground and hit on the cement gourd. On 09/22/21, staff#1 clocked out at 2:25 p.m. and child#1 was picked up at 2:45 p.m.. Once staff #1 left the classroom, staff#3 went to help staff#2 in child#1’s classroom. Per staff interviews, on 09/22/2021 approximately at 2:15 p.m. after nap time, while staff#2 was putting napping cots away, at the same time child#1 wanted staff#2 to hold child#1’s hand, and child#1 was crying and following staff#2.
Based on a video that was taken by staff#5's phone, on 09/22/21 at 2:16 p.m. , child#1 was still is in the classroom. Per staff #5, approximately at 2:30 p.m. child#1 was brought to the office and staff#5 attempted to read a book to child#1 where there is a small rug, books and a child sofa but child#1 did not want to read a book. Per staff#5, between 2:30 p.m. and 2:45 p.m. while child #1 was in the office with staff#5, once the facility office phone rang and staff#5 went to answer the phone, at the same time child#1’s parent opened the office door while child#1 was on the carpet area.
Although the allegation may has happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore at this time the above allegation is Unsubstantiated.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Rohini David, director at 2:30 p.m..
