Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced annual random inspection to the above facility on March 5, 2025. LPA arrived at the facility at 12:30pm, identified self and met with Latanya White, Site Supervisor, who guided analyst on a tour of the facility. LPA provided Site Supervisor with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. Upon arrival, LPA observed 14 infants. This is an infant program which consists of 2 classrooms: Infant Room #1 (4-months to 2 years) with 2 staff and 5 infants, Infant Room #2 (ages 1 -2 years) with 4 staff and 9 infants. The facility was observed to be within the license capacity and limitations. Facility operation hours are Monday through Friday from 7:00am to 6:00pm. All areas identified on the Facility Sketch were inspected. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. The facility has sufficient napping equipment, feeding chairs, and changing tables. Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. The changing table was observed to be within arm’s reach of a sink. Toys were observed to be clean and safe. Cribs were observed to be free from loose articles and objects including blankets, pillows and no objects were observed to be hanging above or to the side of the cribs. LPA observed one napping infant who had turned on their stomach. LPA advised teacher to return infant to their back each time the infant rolls over. Five staff were present in Room #1 and 4 staff were present in Room 2. The cribs have a clear transparent side that allows for constant audial and visual supervision. No infants were observed to be swaddled. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. The center washes linens.
General sanitation was observed. LPA did not observe any infant walkers, baby bouncers, inclined chairs, baby saucers, or any similar items at the facility. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Site supervisor states that poisons are locked in the bathroom. Carbon monoxide detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any

other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. LPA observed labels and dates on all food, including dishes and bottles that are brought from home for proper storage. Trash cans used to discard food have tight fitting lids. Outdoor playground equipment is in a safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. All floors were observed to be clean and safe. All materials accessible to infants were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. Infant Records were reviewed. The name, address, and telephone number of child’s authorized representative is on file. Inspection of required forms was made and documented on the LIC 857. Infant Needs and Services Plan are on file for each infant. LPA discussed the Individual Infant Sleeping Plan (LIC 9227) for infants 0 – 12 months and provided an example. LPA was unable to review staff records during this inspection. The staff records are stored at a main facility located at 2501 Cherry Avenue, #350, Signal Hill, CA 90755. LPA observed that the facility has a current updated infant sleeping log available that notes the time of each 15-minute check, date, infant’s name, initials of staff person conducting each check. LPA observed that infant staff are checking sleeping infants for labored breathing, signs of distress, and position. LPA discussed Provider Information Notice (PIN) 20-24 CCP: Approved Safe Sleep Regulations in Effect, and Infant Safe Sleep FAQs. Children's roster was reviewed and is current. This center utilizes and application Care Connect for Sign-In and Sign-Out. LPA reviewed today’s sign-in/sign-out. Children present were signed in. Disaster drill log was available, last drill was conducted on 01/08/2025. First Aid supplies were observed in the classroom in a bin. According to Site supervisor, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING on 01/31/2023 AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.LPA referred Site Supervisor to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.
LPA discussed the safe sleep regulations with Site supervisor and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep as an additional resource. LPA also informed Site Supervisor of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication. Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the LICENSEE ****, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Site Supervisor, Latanya White.
There were no deficiencies cited during today’s inspection.
