On February 7, 2024, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced complaint investigation for the above allegations. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with director, America Torres who guided LPA on a tour of the facility. LPA observed 11 infants in care with 3 staff.

During the investigation, LPAs Ayala and Perry interviewed Staff #1 (S1) to Staff #4 (S4). LPAs were unable to interview Reporting Party (RP), due to RP remaining anonymous.

RP alleged, "Facility does not properly dispose of dirty diapers." Interviews conducted with S1-S4 disclosed that the toddler classroom does not have a tight fitted trash can for diaper disposal. Based on LPAs
observation, there was no lid on the trash can where diapers are disposed. LPA Ayala obtained photos.

Based on interviews conducted and LPAs observation the above allegation is deemed Substantiated. The preponderance of evidence standard has been met, therefore the above allegation(s) is found to be Substantiated. California Code of Regulations,(Title 22, Division 12 & Chapter Number 1), are being cited on the attached LIC 9099D.

An exit interview was conducted and a copy of this report was provided to director America Torres, along with Appeal Rights. A Notice of Site Visit was provided, Notice of Site Visit must be posted for 30 days.
