On 03/25/22 Licensing Program Analyst (LPA) Fabiola Vasquez conducted a required unannounced random inspection. LPA met with designated facility Maria Corona, Site Director. Who guided LPA on a tour of the facility. LPA observed 2 staff with 7 children in room A. LPA observed 1 staff with 3 children in room B total of 10 children upon arrival. Teacher child ratios were observed to be in accordance with Title 22 regulations. The facility is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

LPA observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There are chairs in the office or a cot is provided if needed. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides AM snack, and PM snack for the half day programs. Breakfast, lunch and snack is provided for the children that are in the full day program All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and water is taken outdoors for children. All storage containers for solid waste, including moveable bins, have covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Storage areas for poisons are locked. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

LPA observed a fish tank in rooms A and B. LPA observed the fire extinguisher to be expired: Date: 3/2021 The facility takes measures to keep the facility free of flies, other insects and rodents. Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. LPA observed a wooden playhouse in the outdoor play yard to be warped with exposed splinters.

Areas around and/or under climbing equipment, swings and slides has cushioning material to absorb a fall; climbing apparatuses has a single upper level. Director states there are no weapons or firearms on the premises. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

Staff Records were reviewed to ensure that a health screening report is on file.
Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

SB792 (Immunization Requirements for Staff and Employees) was discussed with the Licensee. The Licensee and her assistants

VISIT CONTINUATION ON 04/07/22.
At 8:30 AM Licensing Program Analyst (LPA) Fabiola Vasquez conducted an unannounced continuation visit on 04/07/22. LPA met with Abelina Moreno, Lead Supervisor who is substituting for Maria Corona for the day. LPA observed 2 staff with 10 children in room A. LPA observed 2 staff with 11 children in room B a total of 21 children upon arrival. This is a State Preschool program licensed for 45 children which operates Monday – Friday from 8:00 AM – 4:00 PM. There are three classroom, room A operates 8:00-11:00 AM and 1:00-4:00 PM. Classroom B operates an all-day program from 8:00 AM to 4:00 PM Classroom C is not being used at, 34 children enrolled.

The Director’s email address was obtained during this visit. The licensee was advised that email is public information.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following was discussed:
The Licensee shall be present in the home and shall ensure that children are supervised at all times.
Children shall not be left in park vehicles. The capacity specified on the license shall be the maximum number of children for whom care can be provided. Car seats shall only be used for transportation purposes and shall not be used for sleeping. All children in care have the right to receive safe, healthful, and comfortable accommodations, furnishings and equipment. When a child shows signs off illness, they will be separated from other children until the nature if the illness is determined.

Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

Upon receipt of this report, the licensee shall post ANY licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.

The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22 and/or the Health and Safety Code. Please see attached LIC 809d.

Deficiencies that are being cited were observed during a visit on 03/25/22 need to be cleared to protect the children’s health & safety. Deficiencies were cleared during 04/07/22 visit.

Criminal Record Statement
Lead Supervisor, Abelina Moreno reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
